Other than NHS Resident Doctors, do you know of any other examples of where :
A supervisor (obviously more educated & trained)
Is paid less than
Their assistant
whom they are supervising
I would like examples where else this happens please
As parents in UK, who do we want treating our children if they are very unwell
A. Highly trained specialist doctors who have passed all competency exams
B. Someone who has done 2 weeks training in paediatrics as part of their superficial 2 year course
@iDrSunny
The paeds sub-specialty training application process is brutal. The competition levels are ridiculous.
Which is why I’m so angry that these PA roles exist.
Either the exceptionally high standards are necessary to provide safe, quality care or they aren’t.
Wes Streeting
@wesstreeting
can you get involved please
I know you were handed an absolute mess but we need a strong leader to stop this
Unless the regulators are held accountable they will continue with the previous Govts wishes but now you will be held responsible
He laughed at that stupid ignorant comment about doctors always being on holiday 😡
GPs have worked to protect our patients and worked and worked despite burnout till some of us felt suicide was only way out and he laughs about us being on holidays
😡😡😡😡😡😡😡😡😡😡😡😡
Rishi Sunak heckled during his speech.
"The NHS is disintegrating.... 2,500 GPs are unemployed because of your policies... 37,000 Gps will not vote Tory... you cannot employ lesser qualified people instead of GPs...
Sunak: "🤣... I worked very hard in my mum's pharmacy.."
How have 🇬🇧 become the country to promote Quackery over Medical education
Just how are UNREGULATED, unregistered individuals who have done a 2 year UNACCREDITED course allowed not only to interact with patients but perform intimate examinations while paid more than doctors..1/4
Local doctors had raised concerns as early as Feb 2023 !!!!
It clearly wasn’t a mistake
It was intentional
So who acts in these circumstances - they did break the law
GMC🤣
CQC 🫥
Police ?!?
👇
@DrPhilBanfield
has written to NHS West Yorkshire ICB about the damaging, misleading, unlawful ‘It’s a GP practice thing’ campaign. Local doctors raising concerns about the title “Physician” being applied to non-doctors since Feb ‘23 were systematically ignored. We need answers
@wesstreeting
@wesstreeting
tell this patient why they should agree to have a suspected cancer removed By a physician associate with NO medical training or go back to the bottom of the list ? Why are patients being bullied into substandard care in a 2 tier system ?
Once GP led 1°care is gone - it won’t come back
It’s the patients that will suffer
If we don’t speak up to protect the remaining GPs who are being squeezed out of their career, we really won’t have our family doctors anymore
#SaveOurGPs
GP led primary care is being intentionally killed.
A key reason to kill it is because it is actually the most effective and efficient system in the world
If you keep this system corporates can’t make money.
So you have to undeniably shatter it to enable increased activity
Consultants you will eventually retire
If you would like to get quality care in your retirement, please train quality doctors rather than being lazy and training someone who doesn’t have any foundational knowledge
Please be like
@DrNeilGuptaIR
@Gastrografin1
@marklittle_IR
@RBNHSFT
@nhsuhcw
We've had an IR FY2
@nhsuhcw
for 4yrs.
Takes 1month to train them, then an amazing asset for 3months.
I understand benefit of PA, as only need to be trained once.
However about 30% of my FY2s have entered radiology training to become potential future IR Consultant colleagues!
We want our family doctors back
That means those small local practices where the staff know each and every patient and aren’t just an impersonal voice on the phone
.
@wesstreeting
you promised the return of family doctor. We will hold you to it
GP - BIG IS NOT ALWAYS BEAUTIFUL
This is a reminder to
@wesstreeting
@SKinnock
that in General Practice smaller practices often provide better support & continuity of care for patients
Don’t listen to Tony Blair or Lord Darzi if they tell you otherwise
This is our future in 🇬🇧
We will be seen by someone who isn’t a doctor and has done 1 week of studying Oncology-
assessing, diagnosing and treating our cancer
instead on an Oncologist
💔
Feeling very disheartened & hopeless today
Want to diagnose, assess, manage cancer patients?
Doctors:
4-6 yrs med school
2 yrs foundation
3 yrs internal medicine
MRCP exams
4-5 years oncology training
FRCR exams
CCT
PAs:
1 week of oncology lectures
🤔
I understand that other professions enjoy being GPs w/o having to undergo the training/education/exams but do they think about the patients?
A doctor wouldn’t work as pharmacist w/o qualifications but reverse isn’t true
Pharmacists are great at medications, why be pretend GPs?
This annoys me. Bad pharmacists exist. They will make poor decisions.
But one bad pharmacist, or badly supported p'cist, does not make us all the same, or take away the potential value of ARRS.
And yet again, out come all the docs to say how unknowledgeable all other HCPs are.
@NHSDDICB
As a patient I would be misinformed by this advert - Please correct it
Hiding vital information to fool the public into thinking a qualified doctor was seeing them should be illegal
This advert needs to mention
-Not a doctor, Not a GP
-Can’t prescribe
-Needs supervision
It can feel scary when your job role is picked at BUT
Doctors are raising genuine concerns about being replaced by less qualified ppl who get paid more for less work and responsibilities
The Govts have been wanting to break doctors for a long time as they are… 🧵
The bullying of the Physician Associate profession needs to stop
Enough is enough.
Doctors misdiagnose patients from time to time, nurses can make mistakes also.
We are all as healthcare professionals, bear the weight of responsibility that sometimes our errors cost lives.
Each and every trust have to look at where they have hired a PA instead of a PostGraduate doctor and declare just how someone with only 30% basic medical knowledge is safe for patients
PAs please don’t work on doctors rotas - you aren’t doctors & can’t replace one
#PatientSafety
If PAs are not doctors & “cannot be doctors” as per
@RCPhysicians
@NHSE_WTE
@NHSEngland
Why are they on doctor rotas without training?
@gmcuk
This is either a patient safety issue
Or not
“Support good safe patient care” is the stated aim

@NHSEmployers

What new H*ll is this?
So a fresh Science Grad with no clinical skills can just start seeing patients while earning the same as a FY1 Doctor or Grade 5 Nurse 😱
Is this deliberate to further undermine doctors and nurses?
What about patients !?!
#PatientSafety
Fellow patients, are you wondering what all this is about?
GMC has been a regulator of doctors since 1858 to protect patients
They have accredited& standardised medical education to ensure all doctors are at a level of competency before they are allowed to see patients 1/10
1/4: We’ve recently been asked some questions relating to our new position statement, which sets out our vision for the future of medical education and training across the UK. We want to make sure that we’re clear on what this means & the benefits it could bring for doctors &
🚨What new hell is this?
PA in A&E being called an Emergency Physician instead‼️
Just because patients are questioning them- it’s outright outrageous lies now
Please see below how an informed patient was lied to
@parthaskar
@gloshospitals
currently in S&E with confirmation broken wrist. Physician review x-ray without supervision I challenged, asked for consultant supervision advised Physician is qualified to interprete Xraysamd treat without supervision. I'm waiting for qualified medic
Physician Assistants
were supposed to assist doctors so doctors could train/spend time w/patients,not bogged down with paperwork
Changed title themselves to Physician Associates as their ego demanded it-want Drs to assist
Now Generalist Practitioners!
#PatientSafety
So now we know why
@wesstreeting
have been so silent about the scandal of PAs replacing doctors
King’s fund report into RCP shows that they delayed the EGM, to after legislation passed thru parliament-
??corruption??
Why are
@UKLabour
forcing a
#TwoTierHeathcare
on us?
Hi
@wesstreeting
This letter suggests that you fully support PAs and AAs use in the NHS.
And this support was vital to make it pass through legislation.
Firstly - changed your mind since the
@RCPhysicians
debacle came to light?
Secondly - what is that place PA/AAs have?
Goodness
.
@gmcuk
you need to tell us, the public of UK- Why are you supporting AAs to break rules knowing what harm it can do to patients
Why do doctors training to be anaesthetists need 1:1 supervision but after a 2 yrs course non-doctors can work with barely any supervision
🚨PLEASE READ🚨: Earlier today, a video recording of a
@gmcuk
seminar ("Frontline perspectives on integrating PAs and AAs into multidisciplinary teams") was shared onto Reddit. With
@AnaesUnited
cooking something big, a more strategic time to share the video was planned. (1/15)
“Fresh Eyes”
What patients need are trained eyes and minds, not an assistant pretending to be a doctor or nurse!
What kind of new managerial twaddle is this?
Do they really think British public are so stupid that they would accept this nonsense?
#AskForADoctor
Incumbents of the PA role
have “fresh eyes” apparently.
This is transparent, self-serving drivel from
@MFTnhs
Children have fresh eyes. pets in cute dog movies have fresh eyes.
Professionals have training and standards.

🚩 🚩🚩🚩🚩🚩🚩🚩🚩🚩🚩🚩
PAs do a 2 year short overview course with insufficient training on prescribing which is why they are not allowed to prescribe
But they still went ahead and prescribed CONTROLLED DRUGS
This is likely illegal and dangerous
#PatientSafety
🔴 NEW: IT blunder allowed PAs at Calderdale and Huddersfield to "illegally" prescribe drugs inc opiates and sedatives
PAs prescribed oxycodone, codeine, lorazepam, diazepam and midazolam, despite being "instructed they are not legally able to prescribe”
This is the issue- Public don’t understand that PAs aren’t medically trained at all
5weeks of GP training ≠10-15 years of medical education, training & exams to be a GP
That GP can make a mistake but how much more likely is it that after a 2 yr PGdip, the PA would make them
I’m gobsmacked 😶
This is exactly the IEA agenda that GMC are now repeating publicly
Knowing how little PAs are taught, it’s shocking anyone could call it a study of medicine
This is exactly Politics aiming for the bodies piling high
RIP patient safety
@MerseyPensioner
We have an answer from the GMC
According to them, PAs study medicine, just not the same as a doctor who studies it for 5 years.
Whats different? The time and the content.
Isn’t it funny that Nursing leadership has been very vocal in their support of PAs to the extent of blocking Prof Kar & other doctors when challenged
Now that they are looking at replacing CNS with PAs (and RNs with NAs) will Nursing leadership support nurses?
@LastRhian
@caspertown42
he had blocked me - when i challenged his rhetoric
Not to worry- i will do what’s needed for nurse colleagues in diabetes care
What the
Not only are they replacing doctors with barely trained PAs (formerly Physician Assistants) but they allowed student PAs to work on a doctors rota😱
So studied History & then replace a doctor
What r they 💭?
Does no one pushing this care about patients?
#AskForADoctor
From Dec 23 to April 24, there were 72 shifts on doctors rotas that were taken by PAs in the Royal Berkshire NHS Trust
These were in ED and for ENT
Alarmingly, 2 were taken by Trainee PAs. Yes trainee PAs
An excerpt from the FOI response of the shifts taken
I am sure they
GMC need to be renamed as it’s no longer General Medical Council
GaMC- General Associates & Medical Council
GMMC- General Maybe Medical Council
GNMMC- General Non-Medical Medical Council
GMQC- General Medical & Quackery Council
What sounds a more accurate description?
🧵The updated
#GoodMedicalPractice2024
uses the term ‘medical professionals’ as a collective term for doctors, physician associates (PAs) and anaesthesia associates (AAs), rather than listing out each individual role for every reference. We haven't removed doctors from the
Goodness!
Don’t do this
@UCLan
PAs do not study medicine - they do PA studies which is an assistant role
You can’t lump them with medical students who actually study medicine for 5-6years or would you prefer top grade students (who study medicine) to shun your University
@itvpeston
@VictoriaAtkins
@ITV
So ppl are being told to expect some significant tax cuts but doctors asking for a £5 restoration of pay is unreasonable
Maybe some ppl don’t understand what unreasonable is
Loss of access to healthcare in a rich nation is unreasonable
Really important- what an achievement
7 common conditions @ pharmacy first- cost £48/ appointment
Cost of GP appointment- £20 for any and all conditions
NHS has No money for core GP practices so they either have to make staff redundant or close down
We’ve also introduced our important new Pharmacy First service so that people can now directly access advice and treatment from their local pharmacy for seven common conditions. The service aims to free up c.10 million appointments a year in General Practice by next winter 9/x.
@franferner71
I’m a GP not yet 50 but am immunosuppressed.
A mild cold for others can make me very unwell for 5-6 weeks
My family & I are triple vaccinated (I’m waiting for my 4th)
but I am so grateful to all the staff at my practice who are triple vaccinated to protect me and our patients
This is totally unacceptable
Why are non-medics with 2 year MSc & 5-6yrs experience “advising” GPs - are they informing the GP that they are PAs and not in any position to advice senior doctors
Who holds medico-legal risk for following that advice??
#TeamGP
5 weeks 😱 experience in General Practice and they can see undifferentiated patients in 1°care
A first year medical student spends more time in General Practice than this and we don’t even allow fully qualified doctors training to be GPs to locum at practices
“The national curriculum specifies a min of 180 hrs ie 5 weeks” of PA training is spent in general practice.
5 weeks!!!
This figure is hugely concerning & is certainly not long enough to develop the skills to be safe assessing patients w/ undifferentiated symptoms/illness.

Absolutely this 👇🏽
Why is RCGP in hiding?
Why have they not replied to any of the requests for action
Why for years now has
@Parody_RCGP
been the voice of reason
and
@rcgp
the cruel parody
Why are we having to rely on non-GP leaders to defend our profession
1/5
@Parody_RCGP
@rcgp
Where is the defence of the profession from the college , I have seen
@parthaskar
and
@doctor_oxford
both vigorously fighting (thank you) for GPs over these issues recently unfortunately GPs own royal college can only manage to have a meeting in March at some point ! 🤷♂️
Absolutely 👇🏽
Patients want to be known by their GPs, that the GP will notice changes before they become concerning, GP will walk their health journey with them
Continuity of care is the bedrock of general practice & there is clear evidence that it saves lives
But……Politics
GP - BIG IS NOT ALWAYS BEAUTIFUL
This is a reminder to

@SKinnock
that in General Practice smaller practices often provide better support & continuity of care for patients
Don’t listen to Tony Blair or Lord Darzi if they tell you otherwise
47 PAs employed by the trust where 1 incidence of illegal request of ionising radiation scan was reported
This lead to FOI request by a brave Doctor and Paramedic
44 PAs had access to the ICE system to view scans (also used to request them)
1168 illegal incidences were found!
What a failure by
@WorcsAcuteNHS

On- call room for medics with several computers suddenly taken over as Matron office with locks changed overnight
Doctors told to discuss patients & handover in busy A&E as it has computers too‼️
Have
@WorcsAcuteNHS

A few impressions as your former employee:
1. "to create room for medical and nursing colleagues" - while the sign on the door says Matron office
2. "Doctors’ Mess which is nearby" - it is not even inside the hospital.
@RoyLilley
@scottishphil83
Wow that’s just a couple of pounds/hour less than a FY1 doctor gets after 5-6 years at University while working to save peoples lives
No wonder doctors are asking for a pay equivalent to 2008 pay
But bankers (who caused the 2008 crash) are in power & won’t pay for Care/NHS
Why is every new day full of such awful news for patients
Occupational Therapist work in social care
What course few months long is supposed to prepare them to see undifferentiated patients!!
@rcgp
This is what happens when you allow just any quack to practice medicine
@VickiG_physio
@MargaretRoweUoS
Professor, as a physio I’m surprised you aren’t aware but a Registrar is a doctor and should introduce themselves as such at work
This tweet says a lot about you and your chip
@MemeRegOnCall
Had similar calls with specialists and just knowing that e.g patient is worried about leaving his dog home alone and so will refuse admission but we can get the social prescriber to help him out, made so much difference.
The focus only on access & not continuity is a problem
It’s just astonishing that the Royal College of GPs are supporting unregulated unaccredited non doctors who don’t belong in 1°care to replace GPs
NO JOBs for new GPs!
Please 🙏🏽 RCGP members
@DrSelvarajah
@uwa_ie

This isn’t right for GPs & patients
@PaperWhispers
But it shouldn’t matter what her name is- she was the victim
Why are they accusing her of anything?
She was respectful and even indicated that the line of enquiry was not comfortable- any hostess would have picked up on at least that, unless they intended insult.
Very odd but interesting thread with Mrs M making some reasonable comments that Prof Leary really isn’t happy about especially comment that CNS do not know as much as consultants do - why does that offend?
Consultants see the most complex patients cos they know more.
@ShaunLintern
who do they go to for clinical supervision/leadership? I am totally for nurse progression but not at the expense of Drs, we should be a complement not a replacement. It’s not insulting to say that as a nurse, no matter how advanced, i just don’t know as much as a Dr. It’s fact
@profjimspeaks
Really!
Please do tell us
How an unregulated group of individuals are safe for patients
Why 2years course is enough to “work like a GP”
Why RCs, NHSE, GMC have all lied to say PAs aren’t replacing doctors
Also please declare COI
@TheSalariedPA
A flight attendant for 15 years does not gain the right to fly the plane just be cos of 15 years experience

@bmj_latest
@aneezesmail
Haven’t BMA and RCP asked for a pause until regulations and scope is defined?

@JimBethell
@HelenRSalisbury
This is the problem with entitlement- the feeling that they are superior and deserve better than everyone else
Just remember we all leave this world with none of the riches we procure though if ill-gotten it stains your soul forever
@DrBenSpencer
Aren’t you supporting a corrupt government who risked medics lives by selling substandard PPE for greed?
Didn’t you support liars who partied while bodies were piling high?
Look in the mirror - you do not deserve to speak up about any NHS employee or the public
SHAME ON YOU
@drali_g
@ShaunLintern
People are outraged for a short time and then bcos it’s happening so often, it becomes “the norm” since the Govt is busy entertaining us with their clown show every week to distract us from how much harm they have done in 12 years
& there are people still wanting to vote for them
@DeanEggitt
I was recently seen by a rheumatology nurse (18mths late)
I asked her how they were coping w/ staff shortages. She said it wld be ok if GPs wld just start working etc
She then asked what I did and I told her I was a GP looking after the pts not seen by Rheum in past 2yrs
@gmcuk
@LittlePersonDoc
Can you explain why it is taking you 6 months to analyse and collate a crucial survey?
You should be aware that you are a major failure in the basic reason for your existence in the view of doctors and the aware public.
What financial kickback did GMC UK take to push MAPs?
FOI
@gmcuk
Why has GMC agreed to pay Dr Malhotra’s legal costs - is this usual practice?
Does the GMC usually pay for doctors legal fees when they are being investigated?
If not, then can we know why the exception is being made for Dr Malhotra?
@HZiauddeen
This is how the past 3-4 years have felt.
I am not only losing the qualities that made me a good doctor but also the love I had for my chosen profession.
All the yoga/resilience hasn’t helped
The past months have nearly broken me.
To survive I have to walk away from NHS.
On a tangent
But Doctors protested against GMC in 1972 to extent of withdrawal of fees when GMC tried to increase fees to £5/year
As per Bank of England inflation calculator
£5 in 1972 = £57.48in 2024
Why are doctors being charged >£400/year? It’s supposed to be not for profit
And allowed to progress faster than doctors with none of the proof of competency demanded from doctors
Just how has this been allowed?
How are PAs even allowed to be considered to work on a Doctors rota not just in one hospital but in most of them?

Are you saying 5-10 weeks training is enough to see patients in 1°care? Any student who has passed 3rd year medical exams should work in GP with minimal supervision ?
If a doctor in training had said this they would be called Elitist
Why are certain jobs considered beneath a PA role but they are unwilling to do the training (GEM) for the jobs they consider worthy of them?
(*Not all PAs- some do work within their remit & competency)
Shaking right now!
The PA debate in the House of Lords-

“We have to find people from somewhere (to do GP appointments) & they’re going to be some people (Physician Associates) that don’t necessarily go through the 10 years of qualification TO BECOME A GP”

Odd that a consultant is incapable of prescribing 28days of BB when dealing with a patient and has to send a letter to GP to get it done.
Wouldn’t it be quicker, better for patient and logical for a consultant to do the script?
A very stroppy letter from a GP in response to our cardiology recommendation to merely increase the patient's beta blocker dose.
"I feel nothing more than a glorified administrator" the GP wrote.
I am not sure what else we could do.

You have pushed & supported the replacement of GPs (specialists in 1°care) with ARRS staff
Unemployment is a huge issue amongst GPs while patients can’t get care they need
Desperate GPs (at pinnacle of career) have to accept £32/hr
Just received this. “We have just hired a newly qualified GP at £32/h in our urgent care. While I have to get the best deal for our business it makes me sick at the same time”
@neillark
@doctor_oxford
Your argument: GPs after >10years of education, training and competency exams can make mistakes
so let’s replace them with someone who has done a 2year crash course
as they didn’t have a problem in a different speciality
@Xeon4f145d96s1
Only 1000 GPs & will come attached with caveats and more work
Stop gap temporary solution for a few GPs
ARRS is a failure and needs to stop- better to increase core funding so practices can choose what they need
I seemed to have missed this first time round but it is explosive
We need to know who made the decision to push PAs in General Practices and what were their COI
We already know that for a certain Dame it was financial
*Repost due to Twitter malfunction*
Of everything to have come to light so far, this is one of the most egregious

IMO it’s the question of respect

If they have respect for the service, they will behave - wait & see how they behave when it’s all private.

There will be a lot of support from doctors and public if BMA can help with this
