I recall my 12 yr old self in her summer uniform of yellow & white striped cotton dress and ankle socks in her school exam hall. She finished early, and asked to go to the bathroom, but was refused. An hour later the 60 students were asked to file out silently, in turn. 1/🧵

She was refused. Instead, for daring to ask, she was made to get up and leave first, to file past all the other students out of the hall, her blood-stained dress displayed to every student as she filed past them, drips of fresh blood trickling down her legs as she left. 3/n

@Laurenharrhy85 Ah thank you. I feel the same when I think of her too. I have an amazing mother who took school to task for such degradation and we were not refused again (at least in my time there) but it saddens me that it took that. It should never take that.

She put her hand up and asked to stay behind to the end, please? She knew she was sat in a pool of menstrual blood, collected on her plastic chair under her. She had been to the toilet before the exam. Swapped her sanitary protection and double-padded, but it wasn’t enough. 2/n

I am an NHS Worker, a GP “Double vaccinated” Positive CoVid test this evening I don’t want to risk infecting my colleagues or patients. My family are isolating with me, because none of us wish to risk harming anyone else. Isolation is what controls spread, it’s the only way

Female doctors have four times the suicide rate of the public. It must stop.

@rheumipainmask Ah thank you. So do I. Long ago now, but I am saddened to see regressive steps rather than progress being made. Humiliation never improves a child’s behaviour. Why do schools feel that restricting access and increasing humiliation is the correct approach?

@JanettoOz22 @fascinatorfun School and sports uniforms need a significant overhaul! Even recently it took over a season of negotiation with The Board to allow the women rugby players at a local club to have navy shorts for their kit rather than white. A whole season of negotiating?! #dailymisogyny

@billpg Oh no, just retired a year or so ago actually. But was asked to explain their actions to my calm but terrifying mother at a meeting. I have never seen a man squirm so… there was a sense of justice from that meeting.

GPs 'shun full-time work as pressures take toll' I work 3 days as a GP but average 40hrs/week. I get part-time income for this. I used to work 5 days but unsurprisingly burnt out at this rate. Where else is 40hrs/week regarded as PT? #changethelanguage

Staff well-being has become an area to cut costs: access to hot meals, a bed to sleep in, Doctors Mess facilities.. this not only makes doctors feel devalued but puts patient welfare at risk. We are human too. We thrive in Teams. We need to belong. 6/6

@NHSuk Can you please make it clear that these tests are only for use in Asymptomatic patients, not those with symptoms who still require a PCR and self-isolation until a negative result? No wonder my patients are so confused with advertising like this...

Before you complain about the wait for your GP appointment, or your surgery, or the wait in A&E, pause to ask the HCP how many hours of overtime they worked last week. The delay is not because we are lazy, or “part-time”, or off on holidays... there are too few of us. Simple.

My 19th GP Appraisal this week. Can honestly say not one has made a difference to patient safety. Let’s not forget this was Shipman’s legacy to the profession. He’d have “passed” fine Has it wasted hours of time I could have been tending to patients over the years? Absolutely.

Just submitted my 17th appraisal. Someone asked me if it’s been a worth while process. My honest answer is probably no - much better would be to have access to regular peer group support & learning encompassed within this. This is what @DoctorsDistress are advocating.

You have little autonomy; you receive job allocations to areas you have no support in. Families may be separated by long distances for years of junior doctor training. Each move is expensive. The harsh reality of the NHS is, you cannot have a vocation and bills to pay. 🧵6/6

The NHS knocks vocation out of us throughout our training. Continual understaffing and underfunding so our hours are beyond safe limits, patients come to harm, the moral injury mounts up… Your personal life is heavily compromised by your shift work and rotas. 🧵5/6

Where did this idea come from that GPs have to accept abuse as part of their “vocation”? That people who are aggressive or rude to us/our staff should be tolerated, that we cannot take steps to protect us? Is it unreasonable to expect the same respect as any other professional?

@KayteWriteDream I do. I honestly think no teacher should ever be the only adult in a classroom of children for many many reasons. Toileting and such sanitary emergencies is only one of many.

From a Doctor view, losing the Hospital Firm was detrimental to staff & patients alike. MMC changes removed what little support doctors had early in their careers by flinging them far & wide across the UK away from Friends & Family when they are at their most vulnerable 5/6

I qualified as a doctor in 1998, aged 26, after 7 years of university, and have been proud to have worked in the NHS all my working life. I do that for an income that falls year after year, where I face aggression, abuse and a risk of violence every day. (Assaulted x 4) 🧵2/6

I think there has been a massive shift away from Team in healthcare, and that has been detrimental to both the learning culture and staff wellbeing. And this impacts on patient care, well-being and safety as well as that of staff. 2/6

Everyone now works their “shift”, meaning the teaching opportunities and learning events, as well as relationships between colleagues, are hard to foster and harder to maintain. The subtle cues that someone may be struggling are missed or, worse, ignored. 3/6

Today I heard both my cousin and her mother, another cousin, had been admitted with #COVID19 . Sadly my younger cousin died just six hours later. I really have had enough of all this now. RIP

@DrEilidhMaria Need a phone these days, eBNF, SIGN guidelines both on my phone. And consultant connect which saves going through switchboard. The standard book BNF is now far too big to carry around in a pocket, especially if you use the BNFc too. It’s a safety issue.

Each day is so overwhelmed it is typically 12+hrs - yet this counts as just 2 sessions. It would be 3 PAs for consultants. A typical GP working “6 sessions” is FT now

@RoshanaMN I thought my three had chosen wisely as all chose non-medical first degrees. One has realised they have the vocation after all and is keen to do postgrad medicine now. I’m feeling a strange mix of worry, pride and reassurance that they are choosing this with maturity and clarity.

@DrHarryThomas My kids report their obs along with their symptoms when they get sick. I also get told a fluid intake & injury history with any headache, and a bowel movement history with any abdo pain.

@Dr2NisreenAlwan Can we measure the longer-term mortality AND morbidity please? Tragic though these deaths are, each & every one, the sequelae for Covid survivors can have a huge impact on their wellbeing, their incomes, their health needs, all impacting on wider society. This needs measuring too

@ofendlessbooks Exactly. And not just in Primary Care, but hospitals too. Terrifying prospect really as they may be seeing patients who are immunocompromised, unvaccinated or those who haven’t had a good Antibody response to the vaccines. It resembles a massive Darwinian experiment, I fear

As a doctor and an obese person, I agree wholeheartedly. There is too much stigma. Far too many barriers to appropriate care in the U.K. It is the last acceptable prejudice in the NHS, and this needs addressing.
OAC is saddened to hear of Ellen's passing and her obituary serving as a reminder that #weightbias in healthcare can be detrimental. If you feel as though you are not being heard, you are not alone. Your weight/size should never define your healthcare.

They cannot acknowledge that which they choose not to believe in, and those they live with will modify their behaviours to accommodate their views. I suspect this man is totally unaware he has menstruating family members.

@alishawgriff Neither winter uniform nor coats allowed. Even the uniform (archaic for girls of such age) smacks of pre-war era when girls were more likely to have left school than reached menarche. We did get rid of the awful dresses as a result of this though! And a choice of skirt/blouse

@StigersFan Yes, my mother raised it and teacher was made to meet with us to discuss ‘the issue’ which my mother had no discomfort in naming as bullying degradation. The teacher was unable to even say the word ‘period’ himself he was so embarrassed. It felt good.

I know of two fabulous GP colleagues who have quit the NHS in the past two weeks also. It is heartbreaking but I totally empathise. I always imagined I would fight for our precious NHS until my last breath. Now I have an early exit planned myself. #GPburnout #GPcrisis #teamGP

I emailed my MP @JulieMarsonMP and received the standard cut and paste response you highlighted, talking about the AA taper so it shouldn’t not affecting most doctors. 🤦🏼‍♀️ AND despite me explaining I am a GP and signing the letter Dr, she addressed it “Dear Ms Thompson”!!!

Duty Dr today: a typically intense day that is usual right now in General Practice. 47 consults today. Thought I was heading towards one of those days where there would be 65 or 70, but suddenly it all dried up… Not a single call after 4:45pm?!

And this is why my birth plans all said “I am comfortable with whatever action is required to get my baby/babies and me home safely and healthy. Thank you” Birth is messy, painful, unglamorous, hard work for everyone involved; it will often need medics. We need to #tellthetruth

@DrSdeG @flowergirl_lon Shopping around costs too though. In my town, it’s Tesco, Sainsburys, Waitrose and a small Aldi with very limited stocks. When you are unwell or need to make a quick trip, it’s one single shop, often with later hours, that you need, so here it is Tesco that everyone uses.

@timricketts_ I think my Gen were certainly selected for being able to display how much they viewed med as a vocation….I’m now unsure if this is good or not. It’s worrying how many people feel their ID is so linked to work and struggle when that stops for a while….hard lesson to learn

@lcnutt92 Not a practical option for most. School provides more than academic lessons, it is community, friendships, different ways of thinking, new approaches and experiences beyond your family unit. And for many households allows an opportunity for parent to work. But it must be safe.

@DrJessMRCVS @DrSdeG I am so sorry for this awful experience. Sadly far from unusual, I see and hear this far too often in my capacity as a GP, and have experienced similar as a patient myself. I hesitate to say this, but hospital nurses seem generally very reluctant to provide adequate analgesia.

@DrSelvarajah @Parody_RCGP @NHSPracHealth @dave_dlt The longer I do this, a few decades now, the more I feel we all need regular MH support in this role. Normalise it. Rather than gaslighting the profession to be ‘more resilient’, recognise it is tough, demanding, stressful, & working patterns limit normal family/friend supports.

I know of at least one sudden cardiac death in a younger patient who had been telephone triaged away from their first Rapid Access Chest Pain Clinic referral, and was awaiting an appointment for their second referral when they collapsed and died. There will be more, many more.

RAPID ACCESS CHEST PAIN CLINICs Currently referring a person for an assessment of suspected angina - best locally 47 weeks - best nationally 15 weeks (200 miles away) This was & should be 2 weeks, impossible/unsafe for patients & GPs This Government is responsible for deaths

@rachali79 @RoshanaMN I think you are right about closing. I recall as a junior hearing about a doctors’ strike in NZ. All patient care continued, all paperwork stopped. Anything non-critical ceased. Including death certs. Morgues filled in 2 days. Overflowed by Day 3. Govt rethought by Day 4.

@carolmay54 I guess that depends how much we value life/long-term health? Small businesses and those dependent on the s/employed suffer hugely if death or Long Covid occurs. I am unlikely to need hospital as double vax but I’m still pretty unwell and no-one needs this. Avoiding is better?

@AJMattis Thank you. I’m so sad to see and hear it is still happening here today too. I had hoped it was all historical until as a GP I was being requested to write letters for students to have access to school toilets ‘for medical reasons’

@trishgreenhalgh Those are the moments.. A few years ago a man sat in my patient chair “I popped in to leave a card at reception but they said you were free due to a cancellation. You saved my life 3 years ago - the consultant said so! - and 2 days ago I became a dad, so I wanted to tell you”

@Liz_ORiordan As the expert speaker, I would hope your hourly rate at least matches the professionals you are addressing. (And if someone tries to persuade you to do it cheap/free, embarrass them by asking what their hourly rate for booking you is)

@fionamac64 @docib Indeed we are; the whole university admissions process needs a review. Too many of the markers used to discriminate between applicants are simply signs of privilege and opportunity. (DoI: GP, mixed education, independent>state on scholarship, Head Girl x2, DoE, etc. Still wrong)

@DrHFRyan While we are at it, can we do something to limit the “just a locum/just a salaried GP” rhetoric too? The consultant who wanted to speak to “a senior clinician” rather than myself last week has been educated now, but there are many more out there than who view us as substandard

To the anaesthetist (who was lovely and corrected) While labouring! The eye roll should really have been mine
