So another lesson is that even on tissue biopsy when confronted by CD45 positive on flow that are negative for lymphoid markers, review morphology and when suitable consider myeloid sarcoma. Other markers on tissue like CD34 and lysozymes were also positive.

Exactly, I checked with our flow lab and they told that they had CD45 positive cells that were not lymphoid as the clinical diagnosis was lymphoma. I went back to my office to recieve the first batch if IHC and this was myeloperoxidase!

Atypical mitosis is an important features of malignancy. Yet, we need the context, what is the question relevant to the patient that we need to asnwer. However, atypical mitosis is not absolute criterion, they can be seen in a few reactive cells.
