Yes. This experiment on the public to use PAs in the health system, without a protocol, without Ethic permission, without patient consent and without independent monitor, is also a criminal offence.
Who advised Parliament on this?
Dear
@wesstreeting
This is a patient talking.
The current use of PAs in Trusts all over the country will inevitably lead to significant harm and deaths.
A national scandal is just a question of time but many would have paid for this with their lives.
@iDrSunny
@DrInsurgent
I'll keep repeating this - we patients DO NOT want PAs delivering any aspect of our healthcare. The few good eggs can't magically dissolve the enormous safety issues that the role itself poses for us. Please get rid and

The media has a moral obligation to inform the public the truth about PAs. I simply do not understand their reluctance/ Are they waiting for another scandal before they wake up?

This system of using PAs is a massive experiment conducted on patients without a protocol, without Ethics permission, without consent of patients and without independent monitor.
This must be the only example of daily assaults on unsuspecting patients.
I ask for evidence and proof of why the system needs PAs after a PA professor said they are needed.
I get called a bully.
Anyone considered the bullying *read assault* that patients are facing for the system not having asked these questions before?
Hello
@bbchealth
it is time you started to tell the truth about the risks of being seen by PAs when they should have been seen by a doctor. By remaining quiet on this important subject, you are failing to discharge a public service.
There are no jobs for GPs either because the government has forced PAs into surgeries.
Having unemployed highly trained GPs while patients struggle to access GP appointments is a national scandal.
@wesstreeting
all patients deserve GPs.

This mum is deeply concerned that you’re planning to employ a Physician Assistant who will be practising on her son with complex cardiac condition called Tetralogy of Fallot ☠️

This defies understanding. Non-GP doctors are not allowed to work in primary care because they not trained as GPs. But, why are PAs, without GP training, being allowed to function as GPs?
One is forced to wonder of the sanity of
@NHSEngland
@NHSE_WTE
Thank you for the response
@rcgp
. Can you please urgently clarify:
1) Why all non-GP doctors, irrespective of their seniority, are not permitted to see undifferentiated patients in 1° care (with/without supervision) as it’s deemed unsafe without undergoing GP specialty training
The ‘PAs aren’t replacing doctors’ brigade need to stop & reflect on the damage they are causing to the profession.
PAs are absolutely being used to substitute doctors.
Replacing the pilots with cabin crew is a false economy that will harm patient safety & increase costs
The PA idea was an experiment of the public, without a protocol, without Ethics permission, without patient consent, without independent monitor, devised and promoted by people who had lost their moral compass.
One day, I hope they will be prosecuted.
I must repeat again:
Time to STOP complaining

THIS is enough to make anyone's brains explode 🤯 An all sirens flashing red alert alarm ‼️PA student who only did 2 weeks in Paeds quasi-training, gifted an NHS post specialising in dealing with Paeds liver issues. Would you accept a cosplay clown to treat your children??!!!

This senseless experiment is just waiting for a national scandal to happen.
Time for an urgent pause to think hard before further expansion.
@wesstreeting
Please continue to inform the public about this evil experiment on using non-doctors instead of doctors, which has no protocol, no ethics permission, no patient consent and no independent monitor.
Must be legalised assault on patients.
@JujuliaGrace
There are enough Doctors, there just isn't enough jobs being made available for qualified Doctors, the Simon Stevens' americanised health, denationalised systems & "providers" receive money & incentives to replace Drs & use unqualified staff instead
As a cardiac specialist, this is deeply offensive to call someone with 2 years of training a specialist- let them take MRCP and EECC exam and 5 years med school and CCT to be called specialists….
The introduction of PAs has been a massive experiment without a protocol, without Ethics permission, without consent by patients and without independent monitor.
How could anyone support this, particularly, the medics and silent organisations?
The NHS does NOT need PAs. It needs retention of properly qualified Drs & nurses.
We’ve asked this question a million times & have yet to receive a credible answer.
PAs add literally nothing to pt care & are more expensive than Drs to employ. Next add in the cost of
How can trusts defend clinical negligence claims if major procedures are carried out by people without a medical degree, surgical training and, currently, professional regulation? Would anyone really have given informed consent to this? NHS Resolution must surely stop this.

A HUGE 🚩 from this trust
The job advert for a PA in this Emergency Dept - they’ll be the RESUS or PAEDIATRIC DOCTOR of the day?!
PAs don’t hold a medical degree & only do a 2 year course in PA studies
These are the sickest, most vulnerable patients
HOW can this trust do
I think this is breathtakingly unscrupulous from

@DHSCgovuk
& everyone else involved in this project of confusing & bamboozling the public into believing they’ve seen a doctor when they’ve been fobbed off with someone less well trained & experienced. 6/n
I agree. It is far better to hire more GPs than PAs, a practice deliberately denied to GP partners by the incompetent and risky
@NHSEngland
@NHSE_WTE
Time to disband them
@wesstreeting
Quicker & easier for GP to just see the patients themselves
Better & safer for the patients to actually just see GP
AND it's more cost effective
No role for expensive physician associates with a 2 year qualification in PA studies (an unregulated, unaccredited course)
Yes. This is a classic example of the good old British class system. This ethos is still in evidence and reflected in the manner in which MAPs has been promoted without telling the truth to the public.
We should take on this role of information
The audacity to suggest it’s acceptable for working class communities to have physician associates instead of actual doctors.
The working class built the NHS, in no small part to ensure equality of access to qualified doctors, irrespective of class or wealth.
I agree. Furthermore, PAs should be regulated by the Allied Health Professions Council and not by the GMC, a deliberate plan to blur the distinction between a doctor and a PA. Culprits are:

The tide of irresponsible, incoherent nonsense PA "leadership" and PA studies courses have immersed PA students in has been one of the most reckless aspects of the PA project

Hello
@HealthwatchE
you might find this helpful and revise your paper about PAs. It is not acceptable that you should turn a blind eye to what is happening throughout the country. Patients are at risk: this should be interest to you.
Are you telling me that the Department of Health in England deemed the PA role to be:
🚨HIGH RISK to patients…
🚨in ALL medical settings
🚨in 2017 ?!!
Yet DESPITE that, continued with their plans to expand PA numbers in the NHS with NO scope or regulation?!
One of the sensible moves to improve the NHs would be to immediately disband
@NHSEngland
and associated bodies, who do not care about patient safety, have presided over an unhappy working environment, never transparent nor ever accountable + expensive
Thoughts & Prayers with that 1 PA who did not get a Distinction or Merit - the only one in 3 years 😩
But on plus side they maybe working as a “cancer specialist” or a “Physician” in a GP surgery in Bradford- why see a GP when u can directly see a “specialist” 🙄
A core trainee put under the supervision of AAs.
The 'assistants/associates' have become the supervisors.
If that is not basically a reg level or above then what is?
This is outright dangerous

Until v recently (and only after had raised serial complaints) GMC staff were being offered non evidence based healthcare screening in the private sector. Which all drs wishing to practice in the UK had to pay for.

This needs to be stopped ASAP.
But did you also know non doctors are being used to replace doctors right across the NHS?
By 2034, in the NHS, the chances will be that you won’t ever see a doctor.
Is this the NHS
It is very strange that
@NHSE_WTE
pays PAs much more than these doctors. How is it that they can afford the training fees and living expenses in addition to the huge cost of additional posts of PAs etc?
@doctor_oxford
@TheDA_UK
@ParagSinghal09
Junior doctors start their careers on £32,398.
There is not a single Conservative MP who would be prepared to do their jobs for that salary.
If we’re not careful there won’t be a single junior doctor who is prepared to do so either.
The Department of Health and HEE defines Physician Associates as being High Risk of causing harm to patients.
Yes you read that correctly.
And they are in GP surgeries and on hospital wards, unregulated and in many cases, unsupervised.
How have we allowed this?
2003: Physician Assistant
2014: Physician Associate
2023: Assistant Physician Associate
2040 (TBC): Senior Assistant Physician Associate to the Physician Associate in Chief
Meanwhile still can't pay for an actual doctor.
This nonsense about 'different knowledge, skills and expertise' has to stop. PAs have a small subset of the knowledge, skills and expertise of a doctor. There's nothing "different" about it
This has resulted in daily assaults on unsuspecting patients:

It’s almost like
@gmcuk
are making it up as they go along in desperate sophistry to make a political imperative to undermine doctors and provide ‘medical professionals’ on the cheap with totally inadequate training. Physician assistants yes, associates no.
It is obvious that the State is deliberately blurring the distinction between doctors and PA. Before we know it, the NHS would be run without doctors.
Question is: Would
@wesstreeting
and
@Keir_Starmer
correct this dishonest system.
🚨🚨 The GMC CREATED A LOOPHOLE for PAs/AAs to breach safe scope of practice in the NHS 🚨🚨
“So how do we do that? That’s really difficult. The scope of practice says you can’t, you can’t. There’s a VERY NICE LITTLE GREY SENTENCE that says UNLESS you develop local governance”
Having seen the scope of practice document it is clear that
@RCPhysicians
has not acted in good faith following
#RCPEGM

We will not agree to anything short of the BMA safe scope to protect patients and doctors.
Any attempt to impose will be resisted to the fullest extent of the union’s might.
You don’t need to be a genius to see that it would make sense to pause AA & PA training & use the money to increase specialty training places for Drs, which are so desperately needed👇🏼.
There’s no economic argument for AAs that I can see.

But
@DHSCgovuk
@NHSEngland
are duping patients & risking health/life
The Royal College of Physicians was created to protect the public from people masquerading as doctors.
It has no business to provide shelter to people who are not doctors.
Many of us regret this outcome; it means one less person to fight for the NHS.
The NHS is a fantastic institution that has always given its best to patients, the rich and the poor alike.
We are going through a bad patch: thanks to our elected MPs!!
This is the type of person chosen to lead important organisations in NHSE; before this, they worked in business concerned with making profits. Time for a public vote of no confidence in NHSE.
@DrPhilBanfield
YOUR LIFE IN WHOSE HANDS?
UK patients and citizens, there is a systematic and covert effort ongoing to REPLACE DOCTORS with less well-qualified staff. This will lead to deaths.
As the skins of the onion are peeled back the, lies of the PA project are revealed.
The 2 year PA studies role is a dependant role that exists under the supervision of doctors.
Now this crash course assistant role appears to be training its’ own supervisors?
By so doing it would not be able avoid criticism of its use of PA for tasks they have not be trained.
My question to the consultants at
@rno_hnhs
: are you guys not concerned about the risk to your patients?
The paper by

Patients should be reassured that every single GP knows this type of consultation and they take it very seriously. You are not wasting time.

PA leadership asked 135 Physician assistants if they felt their title should change to Physician Associate and 135 agreed.
This title change was inflicted on 65 million UK people by the leadership of 135 PAs.

It is sad and distressing how not only the politicians but also all our regulators have become corrupted. All of them have lost their moral compass: they are unable to distinguish right from wrong.
How does one fight such a situation?
The CQC once criticised on one of our inspections that the floor near the door was dirty (spoiler it wasn’t… just scuffed!) - now when it comes to MAPs & patient safety, CQC appear to be very hands off & light touch. Healthcare in the UK is rapidly becoming deregulated.
The more we go down this rabbit hole the worse it gets.
Apparently PAs don’t need to be supervised by doctors. It can be ‘healthcare professionals’
Consultant PAs incoming in to the NHS soon.
GMC is a joke.
A little knowledge in medicine is highly dangerous.
We are seeing the system wide degradation of expertise in the NHS.

As soon as the legal challenge is over, we should have a vote of no confidence in the

How is this a body that is meant to uphold standards when the standards of those that work there are clearly in the gutter?

Physician Assistants are now in management positions in some departments and they hire “remote locum Doctors” to circumvent the prescribing issue ☠️.
⭕️Absolutely dodgy practice

No, Ma'am
The fault lies squarely on the Royal Colleges,
@NHSEngland
@NHSE_WTE
@gmcuk
and related organisations to quite deliberately establish PA without any idea of scope of practice, supervision, regulation.
And for the lie: PAs are not doctors
I’m sure they are “trying their best”.
I don’t blame individual PAs.
They’ve been set up to fail by “leaders” actively resisting a defined scope for the role commensurate with their limited training, exposing patients to avoidable risk.
It’s a systemic failure to manage risk.
So not only is previously-free protest now criminalised, but it can also result in your losing your livelihood when you clearly pose zero risk to patients or the profession.
