If you don't feel upset and angry that many doctors are going to be unemployed, whilst cheaper, less trained staff are brought in to replace them, I question why? Maybe the powers that be have already won, or maybe you never cared in the first place? Either way it is very sad.

It also struck me there was no insight into how a doctor has done 5-6 med school, 2 years FY, 2-3 CMT and then ST posts with ARCPs, exams, hurdles galore, to lead ward rounds, do procedures etc, but somehow a #PA could achieve the same level of competency after any science 16/x

This is simply unacceptable and wrong. In 2003-2004 the @BMAstudents committee met to discuss how PRHOs and brand new FY1 posts should be allocated. I was there and so were people like @MedCrisis and others. There were lots of issues discussed 1/🧵

At one @RCPL_trainees meeting that they would see "the easy Asthma patient in A+E" - I remember pushing back on this point explaining this was a misnomer in itself - any asthma patient in A+E is usually a patient to worry about! I did wonder how often decision makers using 9/x

Stage managed - you had to submit questions in advance of any meetings, if the PRCP spoke and you disagreed politely you were not able to air that at the time. It was really poor and I articulated this, politely - consequently I was labelled a 'troublemaker' @parthaskar 😂 6/x

I still was unconvinced, and became further concerned after online conversations with two #PAs called Teresa Dowling and Jeannie Watkins. It was clear they were hugely ambitious - Teresa stated she lead consultant ward rounds in geriatrics and Jeannie fully supported 14/x

Doctors were pretty apathetic then, Anons didn't exist but there was a growing disenchantment with how doctors were treated, paid, deprofessionalised etc. The learnt behaviour was to 'shut up and put up' with a very paternalistic approach from the medical leaders of the day 2/x

This example actually saw patients in A+E, but decided to keep that thought to myself! Remember, I was labelled the 'troublemaker'. In addition #PAs would do a 2 year course only, have a science degree before (?astrophysics I asked) and soon be prescribing once regulated 10/x

For the the @RCPhysicians hierarchy - they undoubtably wanted a trainees voice, but that voice was only able to speak when spoken to. It had no freedom to release statements saying what really were trainees concerns, and any meetings with PRCP @DacreJane et al were carefully 5/x

Who had very different ideas of what their tenancy agreement was at the @FPARCP , and in fact the @RCPhysicians had no control over the tenancy agreement in the first place - that was @DHSCgovuk and @NHSEmployers and @gmcuk - it seemed like a mess 18/x

At around this time it was clear bottlenecks were being produced - less and less NTN positions available, with similar issues in FY and CMT years. But, a new "innovation" was on the way - physicians assistants (helping with bloods, cannulas, TTOs) were being renamed 7/x

We must not celebrate the PRCP resigning. It is a sign of state of UK medicine right now. We should hold the architects of this mess accountable by whatever means, & focus efforts on bringing honesty & integrity back by holding the enablers accountable in the GMC, NHSE & colleges

Spot on by @mancunianmedic . My experience in NW Eng & Liverpool is that CEOs, MD/Dep MDs, NHSE/NHSNW all collude to bully & shut people up. And, worse, even when you are proven right about absolutely everything you said would happen, they still try and hide the truth #enablers 1/

Watch it tomorrow if you are a MRCP - just finished. Not enough time given for questions, online questions largely ignored, RCP hierarchy very clearly made minds up. Important people vote and then consider #VONC I feel. It is a real mess #RCPEGM

This is what privilege & entitlement is. Most people would not ride this out - they would resign & apologise not for 'confusion' but because the data was presented in a way that could deceive. When you're the leader of the @RCPhysicians the buck stops with you. Resign.

Before anyone says "Stop looking backwards" I will say that if we don't learn the massive mistakes from the past & be aware of how concerns about this situation (& other situations) get quashed in the NHS i.e. #BeKind weaponisation, we will never make things better in the future

So now we have nurses as well as GPs & junior doctors literally unable to find jobs in a catastrophically understaffed NHS - because @NHSEngland & @DHSCgovuk in their wisdom have chosen instead to pack the NHS with cut-price, less well-trained substitutes. Sheer madness.

I have a previous degree. I have received exam merits every year. I have a distinction for my final medical degree award. I have prizes and a reasonably decent engagement in surgical portfolio crap already. The previous system was poor but this is fucked.

My Dad died last year and him and Mum always taught me "to thine own self be true" - I have always kept that in my mind and always will. Be strong guys, keep fighting for truth

- GI trainees must wait until ST4 to scope but feasibly a PA could do it post a 2 year course - if short of scopers why not train SAS, FY, IMT doctors - PAs not the same as nurse endoscopists (most of them very senior or endo nurses pre scoping) 3/x

The thing is no one is surprised by this - it is always the 'usual suspects'. If it was a GP I'd put money on it being Gerada, or Stokes-Lampard; in medicine it would have to be someone completely disconnected from the front door such as Dacre, Carol Black and so it goes on... /1

It is a massive problem for the GMC, NHSE, many colleges & 'senior leaders' in NHS trusts - the vast majority of doctors have zero trust in them, and so zero faith they will be listened to. I don't know how you restore trust now it is broken, but again don't trust them to fix it

Prof Oliver is right. This is where the focus needs to be - ensuring the ideological politicisation of the GMC (via named individuals and Massey) is undone ASAP. Until this happens, there is no end in sight with this mess or future disasters. Honesty, integrity and trust matters

@mouseter_chef So I felt like a lone voice at times or the one screaming into the void (metaphorically) so I was easy to ignore. I tried to galvanise others but understandably many felt uninvested in the future (was not going to affect them) & did not want to risk being labelled as 'trouble' 3/
