My wife battled cancer privately for 4 yrs so our kids didn’t have to carry the burden. She did everything she could to maintain a normal home and the kids didn’t know until 3 months before she passed.

was diagnosed with metastatic lung cancer. She fought it for 4yrs before it became palliative & we finally had to tell the kids. She went into the hospice yesterday. Those of you who know her know how dignified she has been through this.
It’s nearly 3 months, I’ve held it together for the kids but I still find myself drifting between happy memories & emptiness at night.
I had to sell her car yesterday. Watching it drive away brought a lump to my throat.

So mum thought she spoke with a GP today & said “thank you doctor”. The clinician she spoke with didn’t correct her. The clinician she spoke with was an ACP (quick Google check). my mum felt misled. I’ve said to my mum that asking for a GMC is not impolite.
Honesty is needed 😡
My past two weeks have involved telling my kids that their mother has terminal cancer, signing a RESPECT form and managing pain medications.
She has been amazing through dealing with this 4yrs.
I pray every day for one more night, and every night for one more day.

This is somewhat inappropriate. Hannah’s professionalism and achievement can be recognised but to award her Doctor of the week is not right. First and foremost she is not a doctor…how does your trust consolidate this fact? Have you considered the impact on your doctors?

I started my consultant career just after my wife was diagnosed with metastatic lung cancer. At one point a vexatious anonymous complaint was made criticising every aspect of me but the most upsetting part was the claim that I overstated my wife’s illness just to score time off

I’m getting bored of hearing that drs are specialists & PAs are “generalists”…
Framing "work-life balance" and "being a generalist" as main reasons to choose PA over MBBS in interviews as a golds standard answer hints at deeper system issues.
They also know doctors can work in any speciality and what a GP is, right?
Sunday is Mother’s Day. Yesterday my lad joined in with school activities to make Mother’s Day cards. He was making one for his nan. He was offered an opportunity by the school to do something else but he didn’t want to be left out, he thought he could handle it. He couldn’t. 😞
Facebook keeps showing me memories with my wife. Nostalgic, happy and sad at the same time…..complex emotions for me. It’s been a month since she left us 😢

As a trainee I was spoken to by an ANP who accused me of “stealing” cases

So proud of my mum today. She took my lad to the GP and was clear she was expecting my lad to be seen by a GP……she did have to emphasise that I was a surgical consultant though & anything less than a GP would be awkward 😂

The PA student had 3yr biochem degree & 1.5yrs of PA studies.
He had the audacity to claim he’d be doing my job the minute he graduated. Either he didn’t understand or he was missold

I don't for one moment think I could pass right now. But I think I have the ability. I'd love to be able to work towards an attempt, and it would certainly improve governance if those PAs / ACPs who were working on a medical rota had been examined to the same standard.
5yrs medschool & 12yrs postgrad training taught me the following key things
1. Do the simple things well
2. Common things are common
3. Escalate asap if necessary
4. Personal feelings of embarrassment are irrelevant when patient safety is concerned

PT presents around 3pm. A doctor doesn’t appear to be contacted for support or help, and instead a Facebook group of PAs (PAAUK) consulted instead. PT in HB sent to a&e not until 6pm.

This time last year my wife had a CT & we flew to Spain whilst waiting for results.
When we got back we were told her cancer had spread & she needed more chemo.
My mouth said - “same again sweetie, let’s keep going” & she was a trooper
My head knew that 2023 would be crap
3 months to the day. Nearly the end of term for the kids. I’m so proud of how they’ve made it through. But holiday season approaches & everyone is wobbling.
90 days since she went
91 days since we last saw her smile.
How she will be etched in our memory for ever.

As a consultant if I’m expected to supervise anyone then I need to know my liability. I can supervise junior doctors, I know the process, expectation, scope of practice & what my responsibility is. I also know what responsibility/liability my trainees have…for anyone else….

I think every organisation in NHSE needs to consider that most complaints by doctors are now made publicly through anonymous accounts on social media because despite the official pathways they all either lead to inaction, worsening and potential negative repercussions.

I’ve advised all friends and family members to ensure that when they go and see a doctor that they get a name and a gmc number.
I don’t think the GMC should be regulating PAs at all.
It’s nearly 6 months & the strength they have shown to carry on is weighing on them. Their strength & resilience is inspiring, but heartbreaking when they occasionally have to cry themselves to sleep. All I can do is hug them. That lump in my throat feeling permanent.

I don’t think it is segregation. The drs mess used to be the same as the nurses break room on the ward. It gave a quiet space to complete tasks, share resources, talk to colleagues in other specialties about cases etc.

A recent f/u with patient who was told they didn’t have an infection but was given antibiotics & then told they had organ damage when they didn’t. The worst part was explainin why I was correct & the “clinician” they previously saw wasn’t. Hard to do without noting qualifications

Reflecting on this tweet. These “procedures” start from clinic, decision making, risk stratification, consent, pre-op optimisation, procedure, post-op management, complication management, accountability/liability.
Just being able to “do the procedure” is not enough….

Not everyone has a clear career plan in their early years espicially during GCSE/A-levels. People in their 30s and 40s might want to delve into the healthcare field. However, committing to a 4-5 year course, followed by several years of rotations, isn't always feasible for them.

My mum assumed it was a dr and there was no formal introduction. Apparently the term “clinician” was used in the intro. My mum understands clinician to mean MD after nearly 25yrs of doing research with actual MDs. The onus is on the practitioner to be clear and not mislead

1. Getting into PA training is more demanding than medicine
2. PA training just like medicine but condensed
