@ClareGerada @UKGastroDr @NHSEngland @RCPhysicians @parthaskar @DrLKVaughan @ClareGerada As a GP, I don’t understand what benefit a PA brings to primary care. We have excellent GPAs now. Supervising every patient contact of a PA is a huge task and I might as well see them myself. No-one has been able to explain this to me yet.

@dr_musgrave But none of us knew the risks early on? We knew the medical system in Italy was collapsing under the weight of covid and healthcare workers were dying. I think erring on the side of caution by removing clothes and showering wasn’t too crazy.

@rheumipainmask Plus, we then have to review the consultation, action anything that needs doing and close the task. Does this mean we are then left to hold accountability for the outcome? Quicker and easier just to sort it ourselves and put the money back into GP practices.

@GMCharlatan It still makes no sense even if they had scored well. I did social science degree, then GAMSAT (scored high), then did four years intense medical degree, then took another 10 years before I was seeing undifferentiated patients independently.

@Doc_IonaCollins Again there’s this mention that they can increase the skill mix, what unique skills do they bring? And soon they’ll be able to prescribe, after a 2 year course. I actually have no idea how PA’s could make my life easier, as they require so much supervision 🤷🏻‍♀️

@doclauravater I also became a mum during medical school against much advice! Planned the pregnancy so I could forfeit my elective and then went straight back to finish my final year. Went on to have another two during my junior doctor training. Wouldn’t change a thing!

@djnicholl @medicalmodelbri @bbchw @NHSEngland I honestly have no idea how this works for GPs. We have to supervise everything, which is massively time consuming. Surely easier to do it ourselves? We have GP assistants to do much of the simpler stuff we need help with. Yet to find anyone who can explain the benefits.

@ExplosiveEnema2 I genuinely do feel sad that PAs are caught in the middle, seemingly most trying their best to help patients and support the NHS. The blame lies with the senior leaders who have created and expanded the role without due diligence.

@Parody_RCGP I had urologist write to me to tell me he had told family he did not think the patient needed their DNACPR so I should arrange to discuss this further with them. He was in 90s with significant morbidities! Caused an unnecessary hullabaloo.

@JonnyGucks I thought it was a useful example of how PA’s have been treated as doctors by very senior clinical staff. It may make more people review their previous lackadaisical attitude to the situation. Agreed, the name of the individual was not needed and could have been omitted.

@First_Neptunian @gem1509 I did a degree in anthropology, then did graduate entry medicine, worked my socks off for 4 years, and then, 9 years after becoming a doctor, I finally qualified as a GP. Bit different to a 2 year course.

@doctor_dru_ Yep, as FY1 / 2, i did psych, A&E & GP which were all basic pay. I also had a science undergraduate degree, so would have been very annoyed if having to supervise PAs earning significantly more than me after their 2 year course. TBF PAs do support junior doctor pay increase.

@DrGoblin3 We are reducing workload by using my patient info videos to text these patients (who may not need a consultation) the info to self-care and education on when they may need to have a review. I really don’t know how practices manage without using them! 🤪
