A few years ago I missed a rare diagnosis, delaying treatment for my patient. I felt awful. This week I saw the same presentation and asked the questions I’d failed to ask before. Examination and Ix confirmed Dx. So important to learn from mistakes to improve.
What has become of our NHS that the appropriate thing to do for a palliative care patient deteriorating is to call paramedics and then spend hours and hours in A&E? We all know this is happening, but when it’s your relative it’s just awful.

I had a PA call me about a child with asthma to ask me if they needed admission or not. I asked that supervising GP examine the child, but supervising GP was not on site. PA advised me this was their “bread and butter”.
Big shout out to all those in primary care who are doing an amazing job of keeping children out of hospital at the moment. Our referrals are significantly down and I know GP teams have been working really hard to achieve this, often by phone. Thanks to families too using NHS well

Through the pandemic I have never seen any alcohol in any hospital staff room. When I helped out in adult ICU nurses went to their own cars to take breaks to escape the misery. I never saw anyone cracking open a bottle of wine at work.

Absolutely, in paed gastro I’ve had to think around so many shortages: specialist formulas, ranitidine,Diorolyte, senna, omeprazole. It creates so much work, and annoys families. Diorolyte is a real pain as there’s no prescription alternative. Families have to pay for them. 😢

Other lessons I learnt. 1 Reassess the Dx given by a referring Dr, even if you think they make good decisions. Don’t exclude other differentials too early. 2.when someone attends with GI signs/symptoms, the cause may be non-GI. Being a generalist helps see the bigger picture.

Button batteries can be lethal to toddlers if swallowed. They can cause uncontrollable bleeding. LED light remotes can contain them, please keep them well away from young children.

At the end of 2 weeks they are nowhere near a Keele med student at the end of 4th year placement. Nb I’m not against PAs, I think there is a role for them but not in replacing senior Drs.
TBF this is a slow decline and we have had good support from DN previously. The move to Nursing home has meant no GP visits, and lack of joined up care.

Saw this in clinic this week. Mother gave up breast feeding because a dentist told her she couldn’t breastfeed with an antibiotic. Absolute codswallop, but it was too late to go back. Professionals who advise this have no idea of the damage they do.
A mother told to not breastfeed for 24 hours because she was given one dose of an antibiotic which is completely compatible with breastfeeding. I estimate that could have been checked in under 20 seconds & saved a family considerable distress.

My jaw hit the floor when I heard Matt say that pensions issue fixed for hospital doctors. No, it is most definitely not fixed. This is massively stressful and time consuming for Drs. Pension tax is a huge disincentive to take on more responsibility and work.

I dashed down to A&E recently because the nurse sounded so worried about a child with SOB who “just wasn’t right” despite good sats. Turned out to be severe DKA. With kids recognition of the sick child is so important, expertise (nursing or otherwise) is vital.
Sometimes the things that you just leave to grow on their own turn out the best. The nasturtiums self seeded and they make me smile every time I look at them.
This time of year there are lots items with button batteries in eg musical cards, remotes and toys. Please be aware and keep these items out of reach of toddlers. Wishing you a safe and Happy Christmas. X
