I just realised I am in fact the emphysema of medEd. My V/Q ratio is shockingly high. I am the revision personification of over ventilated, and under perfused. When I was dx with ADHD & dyslexia I remember saying I didnt realise noone else worked so hard to be average
Hi 👋-Doctor’s Assistant here! I’ll do those bloods/cannulas AND chase the things for you.I’ll also summary draft your discharge letters,ready for you to approve/prescribe on,&remind you (and the consultant!)when it’s time for lunch. No one skips lunch unless doing compressions!
If I want an interface between the nursing team and the medical team - that is the nurse and a doctor.
If I want some help doing bloods - that is the phlebotomist.
If I want some help doing cannulas - that’s a nurse.
If I want help chasing things - that’s a doctors
So I had to do an online consult today to request an HBA1C because it’s been almost a year and I have a consultant review coming up… the system is daft, so I decided to have some fun with it.
Obviously.

Honestly… I’m going to have to whip these new f1s into shape.
It’s 1430, I still have one who hasn’t taken a break and the other two think I’m tapped when I point out they’re meant to take two breaks on a 13hr shift.
I love my job.
Arrive to work around 6am, and check in with night F1,who is cool calm and collected,having had a lovely night.
LOVE THAT.
Ask her if she’s had her 3x30 min breaks… no, just 40min or so….
“Off you pop please!”
And OFF SHE WENT!
Honestly, so proud!🥹
@maka1687
Welcome to the club... I will be 34yo starting second year, and my friend is 44yo and the year above. You're already a pharmacist, so you'll seriously ace med. My background in History and cultural heritage isn't much help in med surprisingly! 🤣
Once more for the people at the back….
DISCHARGE SUMMARIES ARE NOT EMERGENCIES.
They can, will, and absolutely SHOULD wait until the medical team have eaten/drank some water/been to the loo and taken their contractually valid break.

Hi - I’m a Doctors Assistant - we are Band 3. We exist to protect training time for Foundation Doctors. As a role, we do a lot of admin, drafting discharge letters, phlebotomy/cannulation/cultures, scribing on ward round etc.
Doctors Assistant: aka F1 Rottweiler.
You can absolutely guarantee I know precisely how to escalate when other people try to dump on the F1 team, and have zero hesitation in doing so.
@weeinstillag3l
@ConkerLover
@Dr_Done_
I like to consider myself the F1 Rottweiler. No one gets to be mean or rude to “my” F1s. I’m a permanent staff member, I don’t rotate, and I know who to escalate to when people pull “that’s not my job” and dump everything on the F1. Not on my watch. 😂

@DrBenLovell
They do. I am one in
@MidYorkshireNHS
- my job as a Doctor's Assistant is to support the junior doctors in whatever they need. I scribe ward rounds, request/take bloods, cannulate, do admin, TTOs, phonecalls, chase scans etc.
@timricketts_
It’s the most important part of the role of you ask me.
we are the ones who the f1s can ask “hey where do I find….” “How do I do…” - cos we never have to rotate… but we are there to support the foundation docs primarily.
@Doc_IonaCollins
We already exist. We are called Doctor’s Assistants, we are £25k cheaper than a PA, do all the non medical clinical/admin stuff (drafting TTOs, phlebotomy/cannulation, scribing, ordering tests requested by docs), and we don’t need a masters degree for it. ;-)
It’s the night before one of my FAVOURITE days in work.
It’s NEW DOCTOR TIME!
I love working with the new F1s on their first weekend… genuinely it’s my favourite - gives me all the feel good vibes and job satisfaction.
Yes,I really am the F1 assistant!
#doctorsassistant
It was not even 10am and the
@MidYorkshireNHS
Assistant Director has already sent out an email to all our doctors reminding them of their contractual rights to breaks.
I really do work for the best NHS trust around.
@IanCarr16285869
you’re a good egg!
Honestly… I’m going to have to whip these new f1s into shape.

I think that F1s especially are so used to being treated like children, bossed around and having their opinions/wishes/objectives ignored in med school/by UKFPO etc and depend on being “liked” for ARCP that they don’t put their head above the parapet for being treated poorly.
@weeinstillag3l
@ConkerLover
@Dr_Done_
I can genuinely say in 3.5 years as a DA, I’ve never been abused by a jr doc. My experience of the relationships we have with our foundation docs especially has been entirely positive,they’re usually incredibly glad to have our experience on systems etc&assistance with bloods.
@Dr_BellaR
This is called a Doctors Assistant. We exist. We cost 30k a year less, and we don’t need a masters degree for that. ;-) We are a Band 3 role in
@MidYorkshireNHS
, but somehow the same role is a Band 4 in
@RoyalFreeNHS
🫣
@RM_Riles
I stopped using “my pancreas chose early retirement” when I learnt that the pancreas does more than just insulin 😂
I’m a big fan of “I have an overachieving immune system” too though.
Ooooh I also totally forgot!
I discovered today that Actual Fully Qualified Doctors are now so young that they never experienced Netflix as a postal DVD service.
My F1 literally googled this because she did not believe that when I was in uni, the internet came in the mail!!🤣
“Civility Saves Lives”
Unless you’re a phlebotomist,who just shouted at me(in a bay full of patients)for asking him to take the bloods which were on the printer but hadn’t made it to the box when he’d lifted the rest 2 min prior.
He refused.
I love my job,I love my job.Mainly.
@DJ3370178658
Oh, I’m 100% serious - been in post since Oct 2020 in Mid Yorks.
There’s a full team of us, with DAs based permanently in Acute Med, Geris, Stroke, Gastro, Resp, Cardiology and elsewhere. 7 days a week!
@weeinstillag3l
@ConkerLover
@Dr_Done_
They meet me,potentially on their first On Call(I only work OOH)and I can help with the frustrating IT issues,show them how where stuff lives on a ward they’ve never been on, make sure they stop for lunch,and try to “protect” their role as the Dr by dealing w/non clinical issues

So by extension,you're saying that the nursing apprenticeship has created lesser nurses than a traditional pathway?Or engineering apprenticeships are turning out unsafe engineers?I'm sorry,Id have to disagree strongly.Change can be good.Widening participation is not just finance
There was no protest, no “oh I’ve just got to do xyz” - she was like “hey cool, thanks!”
Oh, and I forwarded her “The Exec Team Breaks Email” and told her to share it with all her colleagues 😂
@IanCarr16285869
that might be the most widely shared email ever 😂

This. So much this. I had 5 mini bags of Hariono between 11pm and 5am last night. Type 1 diabetes hypoglycaemia meets ADHD dysfunction and sensory aversions… my choice is coma/ambulance or sweets… 🤷♀️
@julesbuddle
Just incase any of your other followers are interested,I got a heated electric throw blanket from Argos a few years back for £60 or so. It costs less than 70p to run for 8hrs on my tariff and keeps me TOASTY when I’m working from home.Heat the person,not the room and all that!
@DrSelvarajah
Think it will be great for widening participation& helping mature students, like myself to train as doctors.
I'm facing having to withdraw from my traditional course after I have a baby,&can't commit to a 3hr round trip commute to uni but also cant move as hub a consultant here
@dannyjnwong
No, it’s a Band 3 Doctors Assistant.
(Or band 4 if you are in some trusts -
@MidYorkshireNHS
for some reason still think we are worth less than our colleagues elsewhere)
@weeinstillag3l
@ConkerLover
@Dr_Done_
This is even more important when you consider that an F1 in West Yorkshire could very easily have 4 jobs in 4 different trusts, with only one or two shared systems.
Let us introduce our Acute Assessment Units Doctors' Assistants!
They are always super busy on the ward doing bloods/cannulas/ECGs & chasing things.
They also draft discharge letters ready for our medical team to approve & prescribe on!

wanted to improve their performance in trainee satisfaction survey and one of their initiatives was the Doctors Assistant role,to help free up Foundation docs to actually do some medicine rather than ward jobs that don’t really require an MBBS.

I did a remarkable thing tonight.
I prebolused for chocolate, using the actual carb values on the packet… and then ONLY ATE THAT AMOUNT OF CHOCOLATE.

@medicalmodelbri
I’m confused why they don’t just recruit a JCF and a Doctors Assistant for near enough the same money. Mitigate the risk, double the body count… get the continuity with the DA… and a JCF is usually a year, so that’s not exactly “rotating”
@Dr_BellaR
@MidYorkshireNHS
@RoyalFreeNHS
We were brought in to basically act as an assistant to the Foundation docs,because they were getting hammered in the trainee satisfaction surveys.Essentially we are there to alleviate the “non medical” burdens you’ve described,and allow the F1/2s to get the training experience.
Excuse me while I boke in a bin.
Disabled is not a dirty word. It’s not derogatory. Being disabled is nothing to hide.
But I swear to god, anyone who terms me a “person of determination” will likely get a very angry and undignified response.
In other news-my (non medical) sister in law referred to F1s as “Baby Doctors” at dinner,and
@uinseann87
can attest to the fact that I just about ate the face off her.

@dr_dru_
@BeccaMedic
Not the point you’re making but also… Let’s not refer to people as BED NUMBERS.
A.) it’s dehumanising.
B.) The patient in bed 2.4 could very well change 4 times during my 10 hour AAU shift. Use the patients name, help prevent mistakes. Simples.
@ConkerLover
@weeinstillag3l
@Dr_Done_
Honestly,it’s one of those things- I genuinely get a lot of job satisfaction from my role.Not enough to put my baby in daycare&do it during the week for baseline band 3,but once a week is fine 😂
We make a genuine difference to the F1 OOH shifts.And that’s a nice thing to have
@waterman_harry
@timricketts_
@amateuradam
Not on my watch. Discharge summaries are not emergencies. Consultants are independent practitioners
more than capable of doing ward rounds without the supervision of an F1 for 30min. At least the ones I work with are!
@Cleverclog67596
That’s always my goal too - I often try to start *before* handover, so I can get the lay of who might go home, what bloods didn’t get done by night team, and what way I need to try to “manage” (I use the term loosely) the cons on WR to see the simple TTOs right after high NEWS.
Today,I graduated from
@qubelfast
after 13yrs in
@HAPPatQUB

2020 being what it is, I've reverted to an archived graduation photo,with one of the most inspirational,dedicated& supportive faculty members I was privileged to have on my side.
@MarieMCole
thank you for everything.
@kerrymc05443501
Hi Kerry, I’m one of the DAs at Mid Yorks, we are very definitely not Doctors. Our (AFC Band 3) role is to support the doctors in doing admin and jobs such as taking blood etc,basically the parts of their job that take them away from assessing,diagnosing and treating patients.
@dralisonj
Determined to help them all realise that feeling guilty about taking breaks is NOT THE VIBE for us.
I’ve spent ten years cheerleading that with
@uinseann87
-now he is a consultant,he’s as fierce a break protector for the residents as I am😂 (do as I say, not as I did I think!)
@DrEilidhMaria
I have a friend (who is a Doctor) whose 5/6yo nephew genuinely exclaimed - “wow - I never knew that boys can be doctors!” after a trip to ED - his only experience of doctors to that point had been 100% female. 🤣
@parthaskar
Ah, thanks Partha!
We have a great team of Doctors Assistants across Acute Care and Elderly Medicine at

@MY_LenRichards
is going to be wondering why our social media is blowing up today 😂
@DrBenLovell
Sometimes the best support Juniors (F1 especially!) need is a cuppa tea on ward round and wrangling off the ward to have lunch at a sensible hour. I've threatened to physically carry one off the ward once.She was tiny,I'd have been fine, but I'm sure HR would have been unhappy.
Much love to all the wonderful doctors I work with in

Honestly, it’s a wonderful role,that gives me a lot of variation in my day,and a lot of self directed working in terms of how I structure my tasks,but ultimately my tasks are generated from the ward rounding docs.The biggest problem is there’s no upward progression opportunity…
So - 1 month into using a GLP1 (not using the name to hide from algo bots) as a T1.
That’s my 14 day time in TIGHT range (42% to start). 7️⃣k.g.⬇️too.
This, combined with HCL is the closest I will ever get to feeling like I’ve got a functional pancreas again.
Total gamechanger!
@parthaskar
@MidYorkshireNHS
@MY_LenRichards
It’s why I hold out hope that Mid Yorks (especially now it’s a teaching trust) could lead the way with a really GOOD model of medical apprenticeship. The current med apprenticeship seems poor but the DA role, combined with part time extended time frame med school? Winner.
@DrJSherrington
I did support work w/palliative care 5+ years back, and “whatever you fancy” mouth care was standard for us. I’ve seen chocolate milk, whiskey,red wine,pimms,vodka coke, the whole heap on mouth care sponges 😂
I feel like I’ve either had lectures or WR with docs who did similar
@EmergMedDr
Thankfully

@RoyalFreeNHS
However, having seen the job spec for the role in the Royal Free, I think Mid Yorks need to take a look at this - we are long overdue a banding review.

Not to mention that the generations who have “survived” this before are about to learn how very different “survival” looks as a pensioner with less physical reserves and lower immunity than the last time they did so. They might not be so “lucky” this time round.
@SukiSu12
breaks are compulsory! Remember, they’d have that name plate changed in a week if you dropped dead tomorrow.Seen it happen when I was a civ. servant -when their spouse came in to say thanks for flowers/support the managers had to pretend that there wasnt a new butt in the seat!
@LindsayBbent
My personal favourite though, was trying to convince the hospital administrator that my QUB qualified, GMC Registered husband, who had been working in NI for 8 years since graduating was not in fact an “international doctor” for the purposes of relocation and NI was in the UK
Did I pre-bolus for 2 slices of toast and a banana, as I put the baby down for his nap, and go to have a (presumed 30-40min) nap myself? Yes I did.
Did he then have a 2.5hr nap causing me to wake to Dexcom having a meltdown about my 3.2⬇️⬇️?
Yes indeed I did.

@NHS_HealthEdEng
This is so exciting for me, as I face the prospect of having to withdraw as a mature student from my traditional med school due to having a baby and being unable to commute 3hrs daily for next 4 years. This might be my saving grace for a career I am passionate about.
So I’ve been on GLP1 for 10 days now.
The results are ASTOUNDING.
My 7 day TIR jumped 80% to 97% whilst my average glucose is down 1.8mmol.
My 7 day TITR ROCKETED from 46% to 68% -absolutely unreal.
I actually need to adjust my ratios tbh, it is THAT good.

Wish we were here for pump therapy Partha... I'm fed up being told I'm "too well behaved" for pump therapy... It's SO hard to be so controlled, yet we get penalised for working so hard.
Bloody hell, that shift was dire.
Why can't people just be NICE to their colleagues? Is it really that difficult? Fed up with how some colleagues speak to my F1 colleagues that I support as a DA. These are our most junior docs, they need support, not tongue lashings.
I can’t even begin to describe what this feels like as a T1D. Insulin Pumps change lives.Ive been pumping for 1year, upgraded to HCL (omni5) 7 months ago,and it is AMAZING. I mean, those numbers. The QOL improvements. Thank you
@parthaskar
for all you’ve done to make it possible
So I am FINALLY getting sent to Pump Assessment Clinic with the view to getting Omnipod👍🏻the bummer is that the wait list is so long that the assessment clinic isn’t until late Dec w/the earliest possible pump start time frame is MARCH 2023.😭grateful,but a bit gutted

Why is it that I can work LTFT from graduation as a disabled doctor, but I can't study part time ANYWHERE in the UK? Why are we still mandating compulsory attendance at 400 person lectures that could be recorded and remote? It's elitist gatekeeping, and nothing more.
