On 1st March Kate would have been 13. Her death was avoidable. We never gave up seeking the truth & applying scrutiny: why Kate died & consequently why so many families were harmed by @sathNHS dysfunctional maternity service. @BBCMBuchanan & @BBCPanorama 9pm tonight - the story.

My daughter Kate died an avoidable death (in my arms). ‘Normal birth’ philosophy played a key part in her mother’s (Rhiannon) poor maternity care. Walk a mile in my shoes Catrin. Kate would be 11yrs old today if safety had been a priority - not “normality”. @DOckendenLtd Review.

“It’s been down to families… pushing against a system that doesn’t want to learn.” Campaigner Rhiannon Davies says there is a “deep-seated culture” of ignoring families who suffer during labour, and calls on Sajid Javid to hold NHS trusts to account.

Remember....when this started with 1 baby in 2009 the family had to fight for years to get answers. When more came forward the trust dismissed them. It labelled me as scaremongering when I proved there were dozens ...now its hundreds. Consider the long term pain caused to so many

Following from what I wrote yesterday … Poppy Russell’s mother “was non-compliant” … that’s what the midwife who failed to correctly record Poppy’s heart rate said at inquest. Verbatim. A labouring woman in extreme pain and at her most vulnerable was “non-compliant” …

Thank you Shaun. Loosing your child is life changing … 12yrs after Kate’s avoidable death, I can still feel her weight in my arms as I cradled her at 6hrs of age as she slipped away … our lives changed forever from that moment. A ‘safe birth’ should always be the priority.

@sheena_byrom @NadineMontgmery You should meet with harmed families about the impact of all this. Physiological or not the focus should be on a safe birth and healthy baby. Sadly there is plenty of evidence promotion of one route over another has killed mothers and babies. I only see you on one side of this.

This goes on getting worse - there are families caring for severely disabled children today and the Trust tried to “dilute” findings of malpractice. Why did no one listen?

We stand with James. United in grief. United in a mutual vision (and hope) of improved & safe maternity care for all mothers & babies. Such an orchestrated attack (on James) is shameful … like the pursuit of ‘normal birth’ which so many seemingly pursue. Walk in our shoes. 💔

How strong was @katieotty on @bbcmtd - telling her daughter’s tragic story. It is so hard to speak to the media, let alone to so clearly convey the devastation of avoidable loss. Very telling about the ongoing culture at @sathNHS which means it remains a lethal place to birth :-(

@Jeremy_Hunt Training does not change attitudes. The primary problem in contemporary maternity care is the primacy of an ideology of “normality” over “safety”. What are you going to do about that ?
