Well said, critical step... Micropuncture guidewire tip is not J-tipped compared to standard 0.035 guidewires, hence it can easily track side branches such as IEA or circumflex iliac arteries leading to RPB if perforated.

An important caveat of this technique is MC might deflect the balloon touch point away from the adventitia breech point. So take different views to recognize this prior trusting your MC position. Illustration attached explain the shortcomings of this technique.

This is a palliative intervention for symptoms relief. The best strategy is to perform percutaneous balloon pericardiotomy by creating a window in the parietal pericardium by balloon dilatation to avoid reaccumulation in context of malignant recurrent pericardial effusions.

Not only volume (Jack I just got my numbers I did >1300 procedures) is that Prairie Heart mojo is no such thing as watch or assist, you do it all. In addition you work as an independent interventional Cardiologist following the attending-fellow hybrid model. I had a blast year.

Mynx good option but it is off-label use compared to Proglide(5-24F) & Vascade(5-7F) both are FDA approved for venous closure. I do not close all venous accesses, I am selective for those are difficult to perform manual compression on due to large pannus/coagulopathy/large-bore.

Insurance not needed with second perclose, if second Perclose needed you just put another one over the secured guidewire unlike Angioseal you don’t have that advantage for re-access.

It is a good habit to leave the J-tipped guidewire in the artery prior to iliofemoral angiography. This will ensure that sheath tip is not buried into vessel wall as injecting dye into it will lead to dissection, which usually occurs in tortuous vessels. Here is an example:

If you have to post-dil the THV, there is the option to perform kissing simultaneous inflation of stent balloon and valve balloon in order not to crush the chimney stent during valve post-dilatation.

1- Deformed stent strut of valve inflow flaring outwards, the best bailout tactic is remove both the valve and sheath as one unit slowly under fluoroscopy & maneuver CW or CCW if you feel any resistance until you reach the arteriotomy site directing the flared strut 12 O’Clock.
