To all medical students and residents and trainees - some advice that was never given to me - things only get busier and the challenges/stress become more the further you advance (residency —> fellowship —> early practice) - there’s a big misconception that life somehow gets easy
As an attending or after residency or after medical school - work hard but also make good habits early on, spend time with family and establish hobbies in order to healthily decompress as these will serve you well in all stages

Patients with medial compartment OA and underwent

had significantly lower joint awareness, decreased pain, improved function, and higher satisfaction than matched TKA patients at minimum 5yr follow-up while maintaining excellent survivorship.
I routinely use 40 mm heads in patients at higher risk of dislocation in posterior approaches in lieu of dual mobility, with modern XLPE wear rates .02-.04 mm a year I feel very comfortable doing a thinner liner and bigger head in these

for publishing it! Sleep is one of the most common problems after TKA without a great solution - what do you all recommend to patients?
Exogenous melatonin did not improve subjective sleep quality or PROMs at 6wks or 90d following TKA. Poor sleep quality was associated with worse patient-reported function and pain. These results do not support the routine use of melatonin after TKA.

Would offer THA here, no data showing losing more weight helps (and some showing it’s worse), off RA meds appropriate amount of time and optimize everything else you can preop, counsel, betadine, vanco powder topical, good closure - that’s what i do in these patients

Very interesting - 40% for KA for OA, which we knows has substellar outcomes - to me this says, they had little arthritis but kept complaining of pain, so finally got scope - scope didn’t help, still had pain, finally got TKA, still more inferior outcomes, more got revised

Great study - 20% reoperation rate for screw fixation! There are specific risks to failure to identify which may bring this down, but still way too high! We should be quick to go to hemi in these patients - no increased morbidity or mortality with it and better outcomes

More clear data that cemented hemiarthroplasties outperform uncemented - yet according to recent ajrr registry data shows the use of uncemented hemis are increasing - have heard lots of “algorithms” on when and when to not cement - EBM clear - cement superior!
An amazing *randomized trial* on Twitter+academia:
112 papers were randomly chosen to be shared on twitter by a group with ~58k followers or to not be shared. Papers that were tweeted accumulated 4x more citations compared to non-tweeted papers over 1yr.
Interesting take on the fellowship decision on net present value - applicants should choose fields they are passionate about and are excited to work everyday - money can’t replace day to day happiness and job satisfaction

I understand there are likely unnecessary tests ordered and procedures done, but when did insurance companies obtain a “license” to practice medicine? Denials for needed meds and surgeries , PTPs with paid doctors not even in the same specialty, etc - will it end?
Thanks for the shout out Dr Shields - cones and short cemented stems have become my workhorse in revisions - these lobed cones are very helpful - sometimes even have to stack cones!!

Cemented fixation for hemis should be standard - if cementless, and I don’t know what that would need to be an option, but if so I think synergy or summit or collared cementless stem is best, def not a wedge in my humble opinion

For the tibia: very dependent on whether or not they put in the screw to unitize the stem and baseplate, if yes = very likely TTO, if not like in this case, I worked the cement, popped off baseplate, removed cement in cone, disimpacted stem and kept the well fixed cone

Definitely agree; arthroplasty conversion after fixation of Garden 1/2 FNFx was lower(13%)in this study than others(~30%); posterior tilt common theme for failure; other studies show no/minimal increased perioperative risk for hemi over fixation - more hemis is the right answer!
