Last week's #PFA PVI case using the @BiosenseWebster VariPulse catheter. Pulsed field goes fast. For you inquisitive minds out there, it can go 23 minutes skin-to-skin fast. Procedure speed is not the primary goal. But it is an important contributor to safety

Full circle for circular PFA! Exactly 1300 days ago we did the first ablation ever with #varipulse and today ⭕️ PFA is back with the world’s first non-study case. Smooth case - quite a bit faster than the regulatory process 😉

Why I love my job-Friday afternoon bailout. 4th proc, refractory VT listed for transplant. Mapped in sustained VT with Impella. Check lower right screen-260 bpm with nice MAP of 66! Thanks @OLVZaalst @EricWyffels for enabling what is needed, cost-constrained environment be damned

4. 3 months ago, I would have considered point-of-care ultrasound for lung disease silly, and POC CT scanning the word that comes after silly. Now, I think we should install a CT at the ER door. It's fast, it doesn't lie and it tells you who is in trouble (unlike SaO2).

6. We need to figure out better why some people have little symptoms despite ugly scans / ABGs, and why even V-V ECMO seems to do little in what should be its ideal use case. Meanwhile, stay inside it is. And wear masks.

1. If you partition ER in a (suspect) COVID / non-COVID area and direct patients based on initial presentation, then 99.999999% of pts in the COVID area will have the exact same symptoms, imaging and diagnosis. There is virtually no grey area. Have never seen anything like it

5. Much to my regret, there were no Mahaim pathways to be diagnosed nor did anyone ask questions about phase 2 vs phase 3 re-entry ECGs (see point 1). I guess those go to non-COVID but given they didn't ask for help there, I'm guessing they don't see those too often there either

2. ER is certainly at risk of being overwhelmed but with good planning and preparation, many things become manageable. Yesterday the ER team would have managed perfectly fine without "outside" help. That's the whole point. Cases are picking up and they are preparing accordingly

@gbiondizoccai Why don´t we take a moment to see what the underlying disease was (if we will ever be told - completely his decision to share that or not). Would it have been identified by screening? Screening no matter how intensive will never prevent all SCDs. And downstream testing can harm.

Here is pacemap in target region. Note long stimulus-QRS for pacemap, weird VT morphology and tiny late potentials only visible on micro electrodes (red circle). Amazed at how far technology has taken us in tackling these cases. Phrenic nerve survived. VT did not.

Athletes in group sports are always present when a team mate experiences SCD. As @simonkjaer1989 showed it makes a difference. Speed of intervention is the only thing that matters. And can we then please start talking about mandatory CPR training for the general population? 2/3

It is a low cost, low effort, super high yield intervention that will impact SCD outcome The outcome of sudden cardiac death is determined to an absurdly high degree by whether you experience one next to someone trained in CPR or not. Simple as that. 3/3 @EAPCPresident

Obviously we need sham control trials for a complex disease entity with QoL outcomes, as @drjohnm explains Meanwhile hard to argue placebo effect for a SNRTc from 600 to 220 ms measured under general anaesthesia. And hard to argue with a patient that does not pass out anymore.

👇 we all need to start wearing masks outside. I’m not talking about the hospital ones to protect healthcare providers. I mean the “simple” ones that protect your environment from you. Once you think about it, it becomes hard not to see the benefit

Useful primer on the benefits of masks. A recommendation from CDC for consumers to wear cotton masks (with guidelines on how to fashion these products from household items) can improve safety and reduce spread and not strain the hospital supply chain.

Today we performed our first (and 2nd) case using the @VoltaMedical machine learning AI system together with @SeitzJulien . Redo after PVI, AF that went to AT with variable CL. All veins isolated during mapping-a classical 🤷‍♂️situation for #EPeeps

Case from 3 mos ago. 55 yom with 1° AVB since 2006. Sudden PR increase to 312 ms, episodes of 2°/3° AVB at rest. No syncope. Symptoms (dyspnea) during start of exercise, resolve after a few minutes. PR shortens during exercise

True, your first case won’t be 60 minutes (that is trainable too). But it’s objective and repeatable at scale. Oh, and steerable sheaths 😊
