Though using a sliding hip screw construct when applicable vs a nail can avoid abductor issues, both at index and for potential future THA conversion. Implant costs are often comparable now though and seems trainees are coming out more experienced with nails than DHS.

Releases are typically away from the bulk of the critical abductors. When are you noting these? 2nd opinions in clinic for abductor symptoms or when going back in for revision for some reason?

It will be interesting to see the poll data from this case for any geographical trends/variations.

Preserved joint spaces aren’t typically advised TKA anyway.

Aside from the manual labor benefits, I actually find it to be even better for fine tuning of the cup, and easily skip broaches (ie efficiency) on the stem side while still retaining tactile/auditory feedback.

Not sure if the index surgery here had any guide wires perforate the femoral head…

Would not remove nail in either case. Enabling technologies can address that, hand-held navigation tools (eg OrthAlign) suffice without the need for high-cost robotics.

Did you consider a longer plate to cover that metaphyseal gap above the TKA? Extended abx prophylaxis and incisional vacs always a good idea as well

Can you share any literature on outcomes of McMurray's osteotomy being superior to THA? Failure of McMurray's ends up with THA anyway, just exponentially more difficult than a primary THA would have been from the start.
