@txsportsdoc With an entity as massive as CMS, fraud, on some level, is guaranteed. Perhaps if they can trim some fat it would allow more funds to go where they should… to those directly taking care of patients. That being said, I am anticipating more of a dog and pony show

I would address the glenoid with a distal tibial allograft. I had a similar case about 6 months ago which I treated with a remplissage and DTA.

@Davembmd The original post touches on this, but it creates a scenario where the “rules of the game” are different for the trainee depending on where they end up practicing.

But perhaps there were some other steps taken I am not appreciating.

I don’t use custom guides, but, if the deformity/bone loss is bad enough I will use a custom glenoid baseplate (ie Biomet VRS). That is in the setting of rTSA of course.

@Davembmd Did they lump adductor canal blocks in with FNBs? I use adductor blocks in my pediatric ACL and patellar stabilization cases and, anecdotally, I don’t know if I have even seen an issue with one.
