To all consultants who have PAs/MAPs working in your dept If your PAs are doing the same work as your Foundation doctors, then 1 of the following must be true 1. Your PAs are working out of scope at the level of a doctor 2. You are not using your FYs appropriately

Countless people with previous undergrad, post-grad, and phDs do medicine. I have not come across 1 who have found their previous degree useful enough to skip 3 years of Medicine. It's not because medicine is 'superior', they are just DIFFERENT courses!!

To all those claiming we should be happy with our pay because we have 'bulletproof' job security with guaranteed pay progression. We have neither.... When applying to medicine 7 yrs ago, never did I imagine I could be competing for a doctor's job with non-doctors...

It must be extremely frustrating for all the 3 in 4 doctors that were rejected from internal medicine training this year, watching how many non-doctors are getting employed in internal medicine and being trained, specifically because there’s not enough doctors.

Even after all this, there will be people ignorant enough to claim they understand my job and my situation better than me. People who refuse to acknowledge the crisis the NHS and its doctors are in. These people are in the minority.

Myth #7 why don't you leave if you don't like it 1. We are leaving. There is a reason waiting times in A&E are high, waiting lists are high, delayed discharges are common. Doctors are leaving.

7. Shortness of breath 8. Hypoxic patient As for training, I received 2 hours of teaching this month. The 2 hours were not specifically designed for doctors. It was for nurses, ACPs and MAPs. They just delivered it to us because no one has time to train us anymore

2/ settling down Specialty training is rotational in the NHS, meaning we could be thrown around anywhere in the region, sometimes country every 4-6 months. So between 25-35, I cannot plan where I want to live, AT ALL, let alone looking for a house or settling with family

The response of @Keir_Starmer to doctors’ concerns about PAs. In short - ‘we don’t care, but the GMC will set scope’. The GMC have explicitly said they won’t set scope. They will keep a list on excel and earn £Millions for that strenuous task. Where are the leaders?

Thread 1/x Mr. Raj, I am sure everyone in the field agrees with you. Also, we all agree that MScGH and MPH degrees should be "free", and global health conferences too. For things to be "free" someone somewhere must pay the bill.

@BecomingaDr Everyone learns different. Don't stress out watching how others study, try to figure out how you learn best. Uni is a more independent learning environment than school, figure out what motivates you, and don't feel guilty about not doing enough work

2. First person I met was @gabs_finn after more than 3 years, and she gave me the biggest hug. My first memory of her is as a workshop facilitator in my first year, and ever since then I've been in awe and admiration of her as a tutor and as a person! #ASME2022

This is worrying, a 2 year PA degree is not equivalent to a medical degree. PAs were created to supplement the work of doctors, NOT fill doctor rotas themselves, trusts and practises are irresponsibly using them to fill rota gaps. This is bad for PAs, doctors and patients.

1/ Important post that PAs are not the enemy. This is a systemic issue started by underfunding the NHS, and perpetuated by the NHS and consultants, worsened by all the Royal Colleges.

Can you please explain how you regulate without scope? If I wanted to report a PA, what's the threshold? What's the scope out of which they can't practise? The only scope for now is BMA guidance, based on that, almost all PAs would be reported?

@yazeedjamil Unfortunately, majority of trainees and junior doctors don't share your opinion, and have seen their training opportunities dwindle and workload increase. Maybe, you utilise PAs appropriately to improve training for doctors, but that is a rarity.
