⭐️Today, we had incredible first year retreat organized by our amazing PD Dr. @milenasuarez and our prior chief fellow @Ashwin_RaoV. 🔥We had fun time reflecting on our journey before and during GI fellowship and sharing appreciation for co-fellows. ✨Also, we listened to rectal cancer survivor who graciously shared her journey and how she transformed her struggles to advocacy miracle. ✅We also had great session with GI psychologist who shared her tips and tricks on dealing with patients and we did some relaxing exercises. This is Volcano Sign of Appendiceal Mucocele 🌋 which is seen as a large, rounded, glossy, balloon-shaped mass 🎈that protrudes from the appendiceal orifice. It moves in and out with breathing 🫁, and sometimes mucus spews💦 from the orifice. 🩺 Clinical Picture: - Asymptomatic in many cases 🤫 - Can rupture💥 and lead to pseudomyxoma peritone 🧫, a life-threatening complication with a 10-year survival rate of only 45% 💔 🧬 Types of Appendiceal Mucocele: 1. Mucosal Hyperplasia🧪 2. Mucinous Cystadenoma🧫 3. Mucinous Cystadenocarcinoma🦠 📝 Management: - Preoperative CT🖥️: Recommended to assess for intracystic nodules or swollen lymph nodes. - ⁠Ultrasound may show an onion-skin appearance 🧅. - Colonoscopy Biopsy❌🔬: Not advised, as the mucosa is typically not involved. Biopsy risks rupturing the mucocele💥 and causing pseudomyxoma peritonei⚠️. - EUS has been recently reported as diagnostic tool ; 💉 Treatment: - Surgery🏥: - Mucosal Hyperplasia or Mucinous Cystadenoma: Simple appendectomy 🔪 - Cystadenocarcinoma: Ileocecal resection or right hemicolectomy with lymph node dissection 🧬 - Differentiatingcystadenoma from cystadenocarcinoma can be challenging preoperatively, often leading to more extensive surgery😕. - Endoscopic Treatment📈 has been reported and may be an option in some cases: 📄 [Endoscopic Treatment Example]( 📚 References: 1.
