Water is used for visualization in cholngioscopy. What happens if you send the irrigated water for cytology? in this study we showed that adding aspiration fluid cytology increases the sensitivity of detecting malignant CBD stricture from 62% to 81%.(1/2).

Clip with a rubber band traction is perfect for ESD of colonic lesions. However, large and bulky rectal lesions benefit from another type of traction. Along the scope traction using snare or grasper allows dynamic retraction and faster dissection in these vascularized lesions.

Our US Experience in 111 colonic ESD with and without Double Ballon (DB) Platform. Procedure time was similar but DB allowed performing colonic resection entirely with ESD technique without resorting to EMR. DB is another tool to facilitate colonic ESD !

A Novel concept in closure techniques is what I call "tissue approximation devices". These are devices which may not necessarily create firm and robust closure as we see with endoscopic suturing devices but approximate the edges well enough for clips to attain a full closure.
