I agreee with this approach. I was always taught that an LA appendage thrombus was a relative contraindication to PTMC, whereas a thrombus in the LA proper was an absolute contraindication. And in the US today, we could also use Sentinel.

What's your ICD at 4 mm-6 mm? Do lots more bicuspid in Pakistan now . Anchor will be much higher than annulus so go by supraannular measurements.

Classic low flow state with CGS. Timely pmbc is key. Oftentimes too little too late esp when shock with MOF settles in. Much like CGS in critical AS pts. Soo many salvage bav pts don't survive when in frank shock.

Agreed my friend. I did the same when I was in the US until August last year. Never did a TSP without TEE and used bayliss. I've moved to Pakistan since. We do about 500 ptmcs a year. TEE is not cost effective and I've had to learn the "traditional" way. It has worked out fine.

Some of the more seasoned operators here have been doing exclusive fluro guided ( they learnt TSP that way) for decades. I must say I have seen only one Pericardial effusion over the past 9 months ( over 350 ptmcs).
