@alanmcn1 First ever tweet-new year new conversation. Let’s spare a thought for the middle aged teachers who are demonstrably vulnerable

@FrontGastro_BMJ @NabilQuraishi 1 The microbiome is very important for health and wellbeing 2 FMT is available for CDI 3 FMT seems safe 4 There are many trials in non-CDI conditions 5 We need to understand mechanisms #fgdebate

@LindaTutty @drkeithsiau @jamesbmaurice Intriguing idea. SCFAs per se have been disappointing in IBD trials so far. We do need to think about combination treatments to properly position FMT in the "age of biologics" though. Good point. #fgdebate

@FrontGastro_BMJ @NabilQuraishi Intriguing concept raised by first UC (Moayyedi 2015) RCT. Hoping to get good data on donors from intensive one donor-one patient approach in STOP-COLITIS #fgdebate

@drkeithsiau @jamesbmaurice CDI is very different from IBD in which multiple treatments seem necessary. Don't know about "turnover time". Real life evidence-India- suggests UC patients benefit from FMT every couple of months. Insulin resistance trial also suggest repeat treatment may be needed #fgdebate

@FrontGastro_BMJ @NabilQuraishi There are more than 300 trials in the world at present in everything from alopecia to nut allergy. Currently in the UK IBD (STOP-COLITIS) and cirrhosis (PROFIT) in trials. PSC and pouchitis promising. sporadic cases for anti-microbial resistance. #fgdebate

@FrontGastro_BMJ @NabilQuraishi DOnt know. There is a lack of inception cohorts. The Gevers study in newly diagnosed kids suggests causality but the inflammation in IBD would clearly change the microbiome profile after diagnosis. Studies like GEM would be invaluable #fgdebate

@bhmullish I think this is very promising for conditions in which the microbiome is likely to be harmed by interventions such as chemo/immuno therapy/antbiotics/stem cell transplants. Would be a great resource. Need to set it up in UK #fgdebate

@LindaTutty @NabilQuraishi @FrontGastro_BMJ I don't think we know enough about the effects of diet on donor stools/recipient receptivity to make recommendations yet but diet/prebiotics are going to be very important going forward. Good point! #fgdebate

@jamesbmaurice 4 RCTs published so far. 3 out of 4 reached primary endpoint. Mechanistics limited at present but clear and encouraging clinical signal. Watch out for STOP-COLITIS #fgdebate
