Hanging up my stethoscope is the closest analogy. A tough job that I’ve been privileged to do. The medical Reg provides you with responsibility in my view like no other job- for that I’ll always be grateful. For the last time as the take registrar-I hand you over. Bleep drop.

After 12 long weeks today I had the greatest gift- the return of my family. Forget all those things you think will make you happy... in reality it’s right in front of you.
This is a brag and I’ll own it. Cos academia is a bloody hard slog.
Cleared over 100 Pubmed papers. Some trash, some fillers but a few I am particularly proud of.
Thanks to all the collaborators/ supervisors/supporters/ friends.
You all rock.
Never forget how much the smallest publication in any journal can mean to someone. Let’s get rid of the IF snobbery and celebrate colleagues success regardless of journal. To all those who publish anywhere- my hat goes off to you!

Followed you for a while Deborah it’s so lovely to see that just because someone has money, it doesn’t stop them being a thoroughly decent human being and standing up for the correct values. Respect.
Manuscripts will get rejected. Grants and funders will say no. That perfect job will be given to someone else. Your idea will be published by someone else. Academics often don’t share their ‘failures’... but it happens to the best of them and very often. Keep on fighting.
I came into my PhD with little more than a case report. I had no research skills ( arguably still don’t).
Research is nothing more than a lot of tenacity and having the humility to ask for help from those with better skills- if you want it enough it will happen.
Week 6. Really starting to miss my daughter and family now. I would give anything right now for my daughter to tell me to put my computer away and play with her...Time with family precious. A lesson beyond covid.
Today I was the med Reg. I take all new referrals through A&E. Usually I get an array of presentations and diseases but not today.... today the only presentation was this...cough, shortness of breath, lethargy, fever, anorexia

I’ve had 8 grants rejected this year.
The 9th one today read different to the previous 8.
It’s never all the roses that is often portrayed on social media but bags of rejections, hurt and bitter disappointments.

With the demands on trainees ( eportfolio, audits, QIPS teaching, research, mandatory training) should trainees be provided with a “PA” per week to do this? Imho no trainee should be forced to do compulsory things in their own time

Me to FY1- what do you like to do outside work? FY1- I like to run, I compete! Me- at what level? School, county? FY1- I ran in the commonwealth games! Get to know the person behind the clerking machine. Some amazing people out there!

I would love a big conference where I’ve never heard of any of the speakers. I think we have reached a ceiling in

therapy… what’s the worse that can happen using new individuals with completely different ideas?
Journals should be given extra points for publishing negative studies or those that show a phenomenon is reproducible. Science needs to be proven over and over again and we also need to know something doesn’t work.
It’s about putting your body on the line. Will try and defend this like Victor van dyke does for his Liverpool team. Sadly for me probably holes everywhere for the attackers to target.

I dislike international expert consensus. (I’ve been part of some myself). Mostly mates with similar interests, ages, stature who sing from the same hymn sheet. IF we want progress we need diversity of opinion and mavericks or we can continue with the club and fail our pts

Repeating and retesting experiments is a vital part of science that sadly is so under appreciated by the big journals. We need to validate previous work.
We need to talk about authorship in
#IBD
trials. Huge respect for the usual names on these but honestly, how are the same names on every single big trial? Do you genuinely do all the work ( which is fine) or is their a team behind you that don’t get the acknowledgement?

In Gastro nurses perform endoscopy to a fantastically high standard. Often better than us. Are there any differences which mean this must be a doctors only procedure? You can still do the pre and aftercare? Asking for discussion rather than to be antagonistic.
Never any shame telling patients you don’t know something. No shame telling them this question is outside my expertise. Important we stay in our lanes and seek answers and help from others. You can’t know it all.
Medical conferences usually are a merry-go-round of the same speakers. I appreciate these are often selected based on academic prowess but sometimes I feel the messages can be delivered by non experts. Do we need to rethink conferences?
Conferences can be quite intimidating for those new to the scene.

Never feel pressured to network at conferences. For some it’s a deeply uncomfortable thing. Instead go to enjoy yourself, be curious, show interest in things that genuinely interests you and let networking occur naturally.
The saddest of sad news. A giant in the field. A great great friend… he inspired me to be better. He looked out for others, was happy for others success. It’s so easy to say nice things when people are no longer here but in this case it’s all so true. RIP my dear friend.

This round of impact factors has shown how ridiculous these metrics are. Time for a rethink. So many ways to artificially inflate them which can mean important science ( negative results/ repetition studies) are ignored whilst we publish yet another guideline.

If you wanna publish to get praise, to seek adulation from peers and be considered a great…
I promise you it just ain’t worth it.
Do it for you, your patients and those internally you wanna make proud only.
There is never a better time to be a role model. Stand up. Encourage. Be positive. Keep up morale. Tell jokes. Check on each other. Let others know you are not giving up. In our

The problem with any significant break from work ( holiday, vacation, conference) is that it’s immediately ruined by mountains of work to catch up on…. How do we solve this.
Be careful using words like precision and personalised medicine around our patients.
#IBD
is still a disease we treat with a sledgehammer and until we understand the mechanisms that drive these diseases I think these words are premature.
Conferences should provide certificates to those partners left at home picking up the pieces whilst we have a jolly. Thanks to all you stay at home partners supporting us.
We know in Crohn’s disease at least that early effective treatment leads to better long term outcomes
It’s why I believe no biologic naive patient should risk randomisation to a placebo essentially prolonging their time to effective treatment.
There is something magical about seeing a patient in clinic accompanied by their wife/husband that have been married >50 years and still worry so much about each other!
