Today as 2024 starts, my Medical Student Finance Loan balance hit £100k...
That sure is a milestone, but maybe not one to celebrate,
I wanted to put down some of my thoughts about the dire situation funding is for Medical Students in the UK
#MedTwitter
1/12
Huge Loss for Medical Students:
From next years UKFPO applications, students publications, posters, presentations, and leadership roles are no longer taken into account for SFP. This could harm the future
#MedEd
and will disenfranchise those who have worked hard toward academia!
We fundamentally disagree with the decision by the UK Health bodies to move the Specialised Foundation Programme into the PIA system from 2025 and will fight for students by resisting this change in any way that we can.

may have seen a few months back uproar about Barts/QMUL med school's examiner eligiblity including PAs.
I try to close the feedback loop best I can on here - lobbying led by myself successfully prevented PAs examining our final years in their OSCEs this week!
An anonymous source has revealed Barts and QMUL Medical Schools actively recruit Physician Associates as examiners for OSCEs, it seems Aston University isn’t alone in its madness.
One of the world's 1st 4-year undergrad medical degree is another concerning

project underway.
Meeting with the project team today for
@TheBMA

@BMAstudents
Concerns: evaluation, student welfare, funding, international recognition.
#MedTwitter
Any more to add to list?
A concerning day for the quality of Medical Education:
A 4 year undergrad medical course is being designed by uni of Buckingham - but to have announced a start date & pilot schools selected for starting these without first assessing the idea/feasibility is reckless!
This month, despite opposition, the 1st Medical Doctor Degree Apprenticeship (MDDA) launched.
@TheBMA
have many concerns - inequity, lacking WP, funding, NHS impact.
Important we don't blame the 20 students starting, it’s not their fault for MDDAs! It's
@NHSEngland
's new 'ideas'
We have an exciting new opportunity available for anyone interested in becoming a doctor. Find out more about the Medical Doctor Degree Apprenticeships below...
Anaesthetist:
"huh, Rob Tucker, that's a plastic surgeon name if I've ever heard one"
*quits MedEd academic with maybe GP plan to fully utilise the pun that 'Tucker' could be in plastics*
Another solution is tuition fee removal for certain courses that aim towards important health & social care roles.
Perhaps this would only happen with a return of service agreements, eg. 5 years of tuition fees paid by gov., then required to do (5?) years in the NHS 6/
I'm still a med student (6/7), with another year to go after an SU sabbatical & a MedEd intercal. So my student debt is going to rise & rise!
I'll add that I have a part-time job while studying to try earn enough for rent (just got told my contract ends in January... Sh*t...) 2/
Currently, it's only the first 4 years of UG full funding available from Student Finance England, leaving students' final year (or final 2 if they intercalate) down to a hugely reduced Student Finance loan & an unliveable NHS Bursary (total max for a year is ~£4k) 3/
Wales for Sept '24 intake is changing this with final year med students able to access a larger (and more liveable) Student Finance Loan on top of the NHS Bursary.
^A solution (one of the worst) would be to allow all UK students to access full Student Loans in all years 4/
Med Students:
I'm running for
@TheBMA
UK Council to be one of 3 Med Student members. Deadline to vote is Monday 8th April - it's by post so check your home/uni addresses,
A vote for me is one for experience in education & being best placed to creating change!
@manLikeTool
Will raise this one, this is definitely why
#MedTwitter
is useful and I love it very much (most of the time!) as had missed this until now
I personally don't mind the idea of returning to service agreements.
If they are optional for students to take or not, with ways to get out of them if needed, and if the maintenance loans/bursary are also liveable!
Any other ideas to fix Medical Student funding? 7/
The Augar Review attempted to start this conversation: "There is a misalignment at the margin between England's otherwise outstanding system of higher education and the country's economic requirements"
The NHS Longterm Workforce Plan really should've addressed this better... 9/
#MedTwitter
A Medical Doctor Degree Apprenticeship with a timeline starting in only a few months, these really are encroaching at a very scary pace.
The funding is a mess, they may not be able to provide WP access supported more than tradition med degrees, and they're untested!
We have an exciting new opportunity available for anyone interested in becoming a doctor. Find out more about the Medical Doctor Degree Apprenticeships below...
Med Students have many allies against the UK Foundation Programme change moving SFP (Specialised Foundation Programme) onto the PIA computer-generated ranking.
Thanks to medical academics across the UK with

@BMA_Academics
& more below:
I don't want to borrow more but it's a solution to the financial stress many have at the end of medical school if able to take more loan, but currently I can't.
...if I hit another milestone of £150k then I really should throw a party!
But surely there are better solutions? 5/
Excited to have been elected to BMA's Medical School Council execs as Education co-chair!
Supporting as the lead student voice for foundation allocation, UKMLA, PSA & degree apprenticeships to UKFPO, GMC, gov. Department of Education and more

Med Students under huge pressure in studies & financially - no matter the task, if it's work then you must pay them appropriately! Glad to see this issue resolved!
Thanks
@BMA_James_Steen
@BMAstudents
An offer to improve the funding for Medical Students for example could also help with the important task of trying to improve the offer of working in health & social care jobs in the UK if we made it a more encouraging offer to study 8/
Please consider joining the largest Medical Student Trade Union in the country that works closely with doctors & academics. And reach out to your reps & the MSC exec if you are doing your own work towards solving the issues all students have 11/
In my ~6 years of med school, I've been involved in lots of Union work - some more successful than others!
I can honestly say, I have trust in
@BMAstudents
&
@TheBMA
- when we all work together as one (decently) funded institution then we do get more wins for students 10/
Burnout in Medicine is very real - more needs to be done & not just talked about!
Good to see med student issues highlighted too - we worked hard to explain our concerns in 23/24, but I hope in
@BMAstudents
next year to get more actual change!
@hlnewey
Please do share to
@BMAstudents
(info.students
@bma
.org.uk) so we can look into this. This sounds worse than awful!
Entirely the whole issue with their being little thought to retention or the welfare of students in the UK Foundation Programme Allocation process.
This U-turn further shows that UK medical schools we're never supported correctly to ever double places - it was an unsustainable and impossible task!
One which helped drive bad changes in
#MedEd
like Doctor Degree Apprenticeships and severe underfunding
Government U-turn on plans to double number of medical students in England. Time for
@10DowningStreet
to cease procrastinating. The expansion was always unlikely? But halting the exodus of doctors through credible pay offers can happen immediately.

There's many fab non-clinical academics who teach, especially in early years, who can make good Yrs 1-3 examiners.
I think this strikes a balance for now, but will continue to raise the substantial differences between Medicine & PA courses. They should not replace examiner drs!
The move of SFP for academic foundation jobs into the same allocation process as other foundation posts remains a challenge we

are working to see reversed, among other reforms to the foundation allocation process. 1/3

The undue stress finalist med students have endured in the last 24 hours shows a greater problem with how the UK Healthcare Service works:
Below explains where BMA have encouraged previous changes to UKFP PIA. Please encourage all Med Students to get Support & give Feedback!
Yesterday, final year medical students received their foundation school allocations. We hope you matched to your chosen school, however we understand many students are unhappy. We've lobbied and will continue to lobby the UKFPO to build a system that works for all students.
Fab to see this issue getting some light - retention of Doctors should be considered at all levels, including before day 1!
Thanks to
@NickTriggle
& Emma from Warwick med school for getting this out there
@BMAstudents
Hundreds of UK final year medical students have been told by their foundation schools that they could only get three weeks notice of what hospital and city they will start working in.....🧵
This is not what everyone wanted - but the best thing about Unions is that we listen to our members & they've spoken!
The BMA can continue to grow stronger to get more for you as doctors and medical students on other key issues, until next April when pay conversation can restart.
Junior doctors in England, thank you for voting in the referendum and making your voices heard. The votes are tallied and 65.97% of you voted to ACCEPT the Government’s offer.
Your committee met this evening to ratify the result. We’ll be in touch soon about what happens next.

please confirm this to alleviate students worries that it will open & why over 2.5 months later than last yr.
It's not a liveable bursary & more is needed to support incoming doctors in final years of medical school!

Yes, do it!
More thought should be made into how UK Higher Education is organised and funded. Part of that has to be about making the offer better for the degrees for jobs we need desperately - it should've been part of the NHS Longterm Workforce Plan
From UKFPO:
“The placeholder groups contain programmes that are still being finalised by the foundation school. Applicants can therefore either ignore them for ranking or rank them last (however this won’t guarantee that they won’t be placed in a placeholder group)".... 1/
Decided to open oriel and check out the jobs so I can start preferencing them and half of them say location is “to be confirmed”. What sort of fresh hell… how am I supposed to rank this now? Absolute carnage

GP tutor (a GP partner) told us she felt 'just one day' of GP strikes would cause havoc.
The gov. have to improve the offer to GPs - in Jan they announced a sub-inflation 1.9% uplift... their pay loss has been comparable or worse than (junior) drs
We have re-entered talks with the Government over the GP contract changes. Whatever the outcome, you will be asked for your thoughts on the final contract in a referendum on 7th March.

Hopefully this tool will be useful for many, especially with a likely election soon (ish) and a (potential) change of government
Will your MP stand with medical students?

@emergingcannula
It's '4.5' years branded - but that's like how many medical schools finish 5th yr at various early stages depending on elective timings etc. Plus the use of summer holidays.
A 4 year UG course would be considerably less & more concern is about how this would implemented uk wide
Announced <10 minutes before all told to have their journeys finished to avoid disruption
I came home from a holiday early to avoid it,
But really: Poor workers likely without phone signal who don't even know it's been called off,
Gov. need to make offers that end all strikes
NEW: This week’s tube strikes have now been suspended.
Londoners and visitors to our city will no longer face several days of disruption.
This shows what can be achieved by engaging with trade unions and transport staff rather then working against them.
A big start: our students want their community to be acknowledged and protected - this is not too much to ask for by a university.
BLSA, and the student body want our name to associate with however we want to use it proudly. As have Barts doctors for 900 years
SIGN PETITION NOW
As you may know a decision has been made by University leadership to rebrand Barts and The London School of Medicine and Dentistry (BL) to QMUL Faculty of Medicine & Dentistry.
To fix The NHS?
- Workforce planning - lifetime of Drs: med school, foundation allocation, speciality training & consultant posts.
- New GP contract with consultation & target to reverse declining practice numbers.
- Re-consider innovation by evaluation of PAs/MDDAs/4 Yr UG med.
Thanks

There's an okay resolution sorted this evening, one that maintains the quality & is still fair - both important! We'll sit double the number of knowledge exams on Friday instead. I'll take today as 'practice'... Very frustrating practice!
We instead need a system that matches students who have an aptitude & enthusiasm for academic work with suitable programmes to ensure a pipeline of future medical academics. We also must make sure that the important input from medical academics remains part of the process. 3/3
@Kavocado
"You are a 5th year medical student who will soon become a resident doctor that has had 35% pay degradation due to inappropriate management of the NHS. Please take a history from this patient called Victoria Atkins."
@DrCiaranKennedy
If there's a difference in entry requirements and if med schools offer both 4 & 5 Yr UG courses then this absolutely could become a 'pay-to-play' situation
Great BMJ article (as always
@Eabhaaa
)
Added a response (link below):
The cost to taxpayers is yet another reason the welfare of incoming UK trained Drs needs to be prioritised for retention. Requires clear communication & appropriate timing!
@BMAstudents
Almost one thousand doctors about to start their careers in the UK have found themselves allocated to so-called “placeholder” roles.
What does this tell final year medical students about how they are valued by their future employer?
@eabhaaa
@BMJStudent
I think the way this thread was reacted to by so many people, then the government's quote is just not true: "The current student finance system strikes the right balance"
On the 6th October 2023 I shared a link on X to a Mail Online article entitled ‘Militant union leader at the heart of doctors strikes is a Labour activist who boasted of charging the NHS for a strike cover shift. I accept that my words were misleading as the subject in question

Students shared their needs to address concerns of power-dynamics, a new cultural agreement to co-creation, a shared student-staff definition of curriculum/curriculum change, clearer signposted student voice processes, and work towards closing the feedback loop.
