I used to think it was dishonest for trans people not to disclose, but I now understand how dangerous it would be for them to disclose to somebody they didn’t completely trust. I don’t know what the answer is, but early disclosure isn’t a reasonable thing to expect

I have only been an adult ITU doctor for 5 days.
However, I would be surprised if anyone is going into an ITU bed at the moment without being ventilated. If he just needed special treatment and particularly close watching, surely you'd put him in a side room with 1 to 1 care.

I think you’re confusing gender & sex. There are two human sexes: Male & Female.
Gender is a list of behaviours (ie. if you’re female gender you MUST NOT like beer, because that’s a man thing. And if you’re male gender you MUST NOT help with kids because that’s a woman thing)

When I was single, if I did extra shifts then the only person who suffered was myself. However, if I have caring responsibilities and work extra, then children/parents/pets also suffer. Therefore it IS more of a big deal.

There are (or should be) precisely zero genders, as gender is a way of forcing somebody into a box where only certain behaviours are acceptable. Trans gender people are trying to escape the box (but not, as a whole, saying that the boxes shouldn’t exist)

The way I make it less bad for my juniors is that we clerk patients together, and alternate who scribes (and do work based assessments when they are clerking)

I admitted a toddler a few months ago because something didn’t feel right and, for the first time ever, we happened to have empty beds.
Child died 8 hours later.
If I hadn’t trusted my feelings (and had fewer beds), that patient would have haunted me for the rest of my career

One of the best jobs I did was a paed cardio job (ie. the consultants hadn’t done general paeds beyond ST3) where, when they wanted another subspecialty opinion, they would double check with the SHOs (ie. Recent experience of general paeds) that it wasn’t a stupid referral

PAs I have worked with have all been pretty good and keen to learn. Agree with MedTwitter that they shouldn’t get training opportunities to the complete exclusion of trainees; but also think that there are enough training opportunities for all
