My grandma died the day after having a TAVI performed by a consult cardiologist. When the valve was deployed a shard of calcified valve pierced her heart causing cardiac tamponade. This was recognised and a pericardial drain placed promptly. Could an ANP do that?

I have had a lot of DMs As way of context, a 🧵 1) Any health professional doing an invasive procedure should be able to deal with complications particularly life-threatening ones. If they cant pt lives are put at risk I trained full time for 3 yrs, did >1000 first opertor

LA for ABGs in *all* patients (apart from critically unwell) has been in the British Thoracic Society guidelines since 2008. Update your practice and don't torture patients. Also think about if you actually need an ABG or if a VBG will do

@sbattrawden 1 thing I'm not over, 3 years on, is that the Med reg rang me from my dad's phone when he died. I answered expecting it to be him, updating me on rugby & instead my world imploded. Also, the conversation started like they were trying to get collateral history not break bad news

@COSMakin @NorthMcrCO_NHS @BMA_JuniorDocs You are right in medical specialists but standard of AE middle grades are dropping and many depend on locums. This is not the way to provide safe and effective care. We cant put #patients and these doctors at risk. #patientsafety must always come first

I am a firm believer in the benefits that ANPs & ACPs bring but this case is deeply disturbing. Firstly because, as already pointed out, the person performing a procedure needs to be able to manage the complications and secondly the damage it causes to training pathways.

@DrLindaDykes When everyone who's worked there and moved on describe it as toxic When no amount of money is enough to go and do a locum shift When you're labelled as the problem because you're a trainee so that's easier than addressing any of the actual issues When you cry on a weekly basis

As an F1 I made a formal complaint about a surgical consultant. They were a notorious bully - shouting at juniors on ward rounds, belittling them in front of patients. They sent the most unprofessional email I have ever received in which they threatened none of the juniors 1/n

24 hours ago I got a call from an ED to tell me that PapaG had been taken there in cardiac arrest and that resuscitation efforts had not succeeded. I feel completely lost in this whole thing. 😞

In a new and unexpected twist of being LTFT I've been told that I'm in a job share (I'm not) and therefore won't be paid for all the OOH and weekends I work but a proportion of them. I have highlighted how completely wrong this is and am waiting for them to agree with me. 🤦🏽‍♀️🤦🏽‍♀️🤦🏽‍♀️

When she was alive she danced on tables, beat all her grandchildren (all of drinking age) at tequila slammers & once was in a hot tub with 8 naked men... she was also baffled by pyjamas and asked if young people had thought of sex to keep warm instead!

@irishayesha Italy is definitely an example of a country doing it right. No such thing as mask exempt. Green pass required to eat inside restaurants or enter museums etc. People still taking covid seriously.

@NHS I can't believe this is even a question. At a time when morale in the NHS is at an all time low suggesting that they shouldn't be allowed to have fluids whilst not directly involved in clinical care is ridiculous. Dehydration does not a safe Dr/HCP make.

Nurses are amazing but I'm a doctor and I am incorrectly called 'nurse' frequently because I'm a woman. It doesn't happen to my male colleagues.

@silv24 @noodles_nood Reminds me of being the ITU 'reg' in a small DGH as an ACCS ST2 when the senior nurse for the night would be disappointed it was me & that my being there as the doctor for the night was an issue because I 'wasn't airway trained'. Had to deal with 6 months of similar nonsense.

Also please don't use the person's phone to make a call to family members... the night my dad died the hospital rang me from his mobile - I answered it expecting an update on the rugby match only to be told they'd stopped resuscitation efforts

If you ever have to ring a family in the middle of the night please please be able to tell them how their loved one is now. Because answering I'm not sure at that moment really isn't helpful.

@em_j_b No but it does remind me of a surgical F2 (who a few weeks before had been in ED) trying to explain to me how to manage a patient's high BP (elevated due to pain secondary to severe pancreatitis). I declined to prescribe amlodipine and opted for analgesia instead!

@silv24 @noodles_nood This was also a unit where highly skilled EU nurses weren't allowed to cannulate patients because some of the senior UK nurses didn't want to learn how to or to get everyone trained. When I asked one of them why she said it was because she was worried about cannulating an artery

Wtf is a surgical assist? I stayed late when my patients needed it to ensure thorough handover to relevant parties. I struggle to believe that an F1 in their 4th week would be essential to any surgical case when at least a CST and SpR also available.

I hope that the experience forced the consultant to evaluate his behaviour. I stood up to him because I couldn't bear the way he was treating my peers. I had no skin in the game and was able to do so. It's so much harder though when it's your training and career on the line. /fin

@Dr_DeanS A great example of this is when doing the 7am obs before handover and they're the same as they have been since midnight but because it's almost handover it's now a problem (which will probably be rectified by their morning antihypertensive meds 🤦🏽‍♀️)
