Acute delirium? Get history, med rec, vitals, physical exam, labs, urine in an hour. Not a few day wait for PCP. Not 12 hrs in a crowded ED. Uptriage only if needed. Also much cheaper for most pts.

Vascular dementia = stepwise decline? No!
That was a misconception that got codified into a definition. Since redefined, but damage was done. The majority of vascular dementia has slowly progressive decline in cognition. >1/2 of all patients were misclassified
'Extremely controversial for 2 reasons
1) patients may be diagnosed with Alzheimer even though they have no symptoms and may never develop them
2) the majority of the panel received $ from the Alz Association or the drug industry, which stand to benefit from the guidance'
