💯. These little cards are always next to a $5 bottle of water. I’m pretty sure they’re grifting: the housekeepers are often illegal immigrants being paid slave wages, they want to hire even fewer of them by cutting down on bed and towel turnovers. And yet they have this self-righteous little card asking me to facilitate their con.

I’ll tie this to a post @BowTiedBroke made about the terrible quality of nursing homes. Broke, Medicare has decreased payments to physicians by about 50% over the past decade. For instance, if Medicare paid me $100 in 2015 to read an MRI, they’d pay me $50 today to read the same MRI. And while they’ve squeezing medical professionals, @DOGE has been demonstrating the limitless scams depleting money that could have gone to hiring incentivized and attentive caretakers and building wonderful facilities for our aging family members. Instead these clowns use their power to scam everyone around them, creating untold levels of second and third order suffering. There’s much to do to fix this. It certainly can be fixed. But the first thing to do is pry every ounce of power from grifters and never let them touch it again.

Obvious now that the issue for the Harvard intellectual class has always been control. Tasked with reforming healthcare, their first step was to deal with those pesky independent primary care physicians by creating a system that would force them to sell to large health systems. The large health systems would be the MBA run non-profit that advertises in the Super Bowl , is paid multiples for the same service compared to a small independent practice, and routinely sticks the uninsured with insane list prices AND then sues them for payment. But they do have quality improvement teams that track meaningless metrics, and lots of well paid MBA admins to pull the trigger on $100 million EMRs, so there is that. I’ve changed my mind on NIH indirects. Even 15% is taxpayer theft. Let’s lower it to 0. Give the full award to researchers engaged in meaningful research and hospitals can negotiate with the researchers directly for “indirect” costs.

@CoffeeBlackMD Yeah man. Sick enough to go on a vent? Even for a middle aged person, it’s weeks of work and therapy to approach a previous level…maybe. If everything goes great. Elderly…well, probably never. This is a discharge to a SNF. Which is likely permanent.
