After that , the evidence doesn’t support intervention but the argument is nuanced , the debate is age old. One thing I would not do is discharge him from clinic on preventating therapy which sadly happens under NHS pressure

@realarainmd @TinoCostantini Yet to try my first JSBT but conceptually I think POT is necessary whenever you stent across a decent SB as the MV will taper down. So either you risk overexpansion distally or under expansion proximally , even if well apposed

@DrWhyWho @realarainmd @mmamas1973 The categorisation of what’s high risk is extremely variable which may be part of the problem. Let’s hope BCIS CHIP3 provides us with the evidence in high risk PCI cases.

@mmamas1973 @Sudanheartgroup Is there a way to find out if higher volume imaging use is improving outcome as expected ? Or are we taking the IVUS/OCT out (we are 60%+ at our centre) but not really utilising the information well enough ?

@DrBIqbal @realarainmd @agtruesdell That’s what I was wondering as DCBs are much bulkier. I guess you also have to use Paclitaxel DCBs only as they strongly advise against any further instrumentation after Selution. Surely will give #JSBT a try!

@mmamas1973 @DrLKVaughan @o_matthias1 @DrSelvarajah @iDrSunny Hard agree … just imagine how efficient it would be if there were enough ward clerks and Doctors assistant doing those jobs. Patient , trainees and the system as a whole would benefit so much.

@SyedYNaqvi1 Getting much better prep with proper scoring balloon like Angiosculpt. Avoids medial dissections like cutting ballons such as Wolverine but sorts out the usual fibrocalcific tissues when DEB is the intention
