@Aidan_Baron Catastrophic bleeding in smaller regional hospitals. Like ECMO, it’s something ED docs can be taught to do quickly while the consultant is coming in from home to handle the problem definitively. It’s just an internal tourniquet.

@mcmack84 Tpa. ECMO. 3rd line drugs for VF/VT. Obviously not everyone needs to go to the hospital. The problem we shouldn’t be accepting is that most hospitals don’t do more than EMS.

@GoodishIntent Had one of these the other day, came back as VRSA. Ended up the lab messed up. When the culture was re-run it was MSSA. Spent a good two days wondering if we had a case report on our hands.

@critconcepts That’s why I stopped playing w TCD as much. Some folks you just won’t have a window on no matter what you do and it’s frustrating.

@DGlaucomflecken @docfortots Yeah, if they’re not making money. PE isn’t gonna go after them. They aren’t altruistic. Certainly arent going to buy them, just wait til they fail and then come in.

@grahamnichol @DrHowieMell @Abiomed We do impella and ECMO at my place without surgical or perfusion backup. Agreement w university hospital for complications. But allows us to save more that would otherwise go without.

@kane_guthrie @jducanto I’ve used them consistently for years and never had this issue. Definitely not the norm. I’ve seen lots of good advice on this thread to try though. Good luck.
