@ross_prager In the hospital setting, there may be an additional layer for people that get admitted through the ED. I wonder the weight of bias provided by the initial impression given by the Emergentologist (like me) to the admitting team.

@thomaswoodcock @icmteaching @ThinkingCC @ross_prager @katiewiskar @KiranRikhraj Very kind. I learned a lot about the lymphatics from your book. Thanks! It is highly recommended if anyone hasn’t read it.

@IM_Crit_ @ThinkingCC Interesting, thanks. Every sick patient has their own dysfunctional physiology phenotype. Targeting generic hemodynamic targets probably isn’t the best strategy.

@ross_prager @PulmCrit @ThinkingCC @katiewiskar @KiranRikhraj @G2Disrupt Great Ross. For bubble study: if bubbles arrive to LV in 1-3 beats (usually <3 beats) it’s intracardiac shunt. If bubbles arrive 4 or more beats, it’s likely a vascular related intrapulmonary shunt (bubbles don’t get filtered with real shunt, but takes a while to get to LV/LA.

@IM_Crit_ It’s no wonder we forget things people figured out in the 1950s or even a hundred years ago.

@drmohansai @ThinkingCC @jsemccm 3/3 The brain should never have diastolic velocities this low no matter what the systemic effect on the Doppler signal is unless there is a big problem.
