Doesn’t help with adhesions but seems to really reduce the recurrence of collections. One patient developed adhesive peritonitis from the evil CSF humors (!!) but a VA shunt worked for her.

I’ve had 5-6 patients in the setting of tumor, trauma, meningitis, SAH, etc. re-exploration doesn’t offer sustained value as the adhesions promptly recur and loculated collections recur.

@ElswickClay @AJGrossbach @PaulParkMD1 @KhoiThanMD @NaderDahdaleh @AliBaajMD @spinesection I’ve got a difficult, non-compliant patient like this bouncing in and out of the hospital as we speak. With that cord compression and signal change I don’t think there’s an option not to. I would make it a multi-disciplinary project and protect yourself medicolegally. Bleh.

@ElswickClay Those look like old threaded cages - like LT cages… high bulk modulus, as Benzel said, “round peg in a square hole” — subsidence is almost a given. They’re tough to extricate in my experience; I’d probably favor P only with definitive decompression and instrumentation at L5-S1

@ElswickClay @KhoiThanMD @MGalganoMD @docclift @AliBaajMD @NaderDahdaleh @drmikeselby I agree with posterior only down to T2 with decompression C7-T1; MRI after… he’s unlikely to need additional ventral decompression. I usually get a few point of fixation in the fusion mass maybe C4-T2 but dealer’s choice
