To me robot vs open is secondary issue,less controversial. My question was about lack of exposure of modern trainees to open surgery, as they mostly only see MIS now. There will always be need for open sx : massive kidney , level 3 thrombus , trauma, some recon , some prev sx,etc

For DaVinci Xi : useful tip for “open Hasson” technique users for 1st port access ( non veress needle users) : the 12 mm robot port enables larger incision and then insert 8 mm reducer for instruments.

Totally impossible to do robot here: lean patient with massive abdominal distension from mass . It wouldn’t be possible to access instruments and no working space .

Size is not an exclusion criterion for robotic surgery. I have done up to 30cm renal mass robotically & the patient was ambulating immediately & went home next day. No offense to open surgeons, but open nephrectomy almost never necessary (<1%) and eventually will phase out.

My 22 yr old pt , obese, high BP, passed fresh blood via urethra every time when passing stool ( 2 month ) Cystoscopy /scans NAD. now lost 10 kg and normotensive ...Bleeding stopped! presumably bleeding related to very high Bp with straining . Any other opinions?
