Back on the COVID-19 service. We have now seen a handful of patients with 2 negative NP tests (Cepheid) 24 hours apart with positive BAL testing. Anyone with a similar experience? Confirms problematic test sensitivity and importance of clinical suspicion for COVID19.
It has been a tough day. I just said goodbye to my sweet Krewe. He was not even two years old. He lost a battle with retroperitoneal neuroblastoma. This one really hurts as he was such a sweet dog. Oh, the joy he gave me over the past two years! Peace, Krewe.
Most common questions on rounds today: 1) is the COVID-19 vaccine safe? 2) Dr. Baddley are you recommending it? Important for ID Docs to understand the safety/efficacy data and spread the word.
Happy that my oldest kiddo graduated from University of Georgia today. Now she’s off to Physician Assistant school. Glad she will be a healthcare professional.

My daughter, a Physician assistant, just called me about a patient she saw with an infected wound growing ESBL Kleb. She said “Dad, I think we need an ID consult and a carbapenem…” proud dad moment
Just saw a patient who finished a 5-day course of oseltamivir for Influenza A diagnosed by RVP. Now acute dyspnea, confusion. Classic post-viral bacterial pneumonia, right? Wrong- SARS-CoV!
While many people (including me) and businesses are excited about starting to open again, let’s be careful. Today my business was booming on the COVID consult service:

Need some positive vibes from Twitter. My 1-year-old Cairn terrier (Krewe) was found to have a 12cm retroperitoneal mass, a neuroblastoma. It’s wrapped around the aorta, so no surgical option. Now on vincristine/doxorubicin. Sweetest dog ever
Big day today. Finally finished compliance, billing, HIPAA, security, EPIC, IRB and sexual harassment training at my new institution. Today I get to see real patients on the transplant consult service!

I have been lucky to serve as a part-time physician in the VA healthcare system. I have leaned a lot about patient care, leadership and teamwork. I love Veterans- they are truly special. Today is my last day at the VA after 22 years. It has been rewarding but time to move on.

Are people using combination therapy (lipid amb plus azole or echinocandin) for treatment of mucormycosis? In addition to debridement I tend to stick with lipid amb alone per guidelines

Single high dose lipid AmB for cryptococcal meningitis. The reality is that repeating the initial clinical trial in other populations is unlikely. We need to make choices based on safety and science
