It’s been 12 months of me waking up consistently around 4.30am, starting off with a gym workout. I can’t recommend it enough for people to sleep early and wake up early. Your productivity and wellbeing will improve so much.

Can’t keep up with all the shortages. Putting patients at harm and takes up a huge chunk of our time trying to resource alternatives
The NHS is facing a shortage of drugs, including anti-depressants and penicillin.
The number of drugs in short supply has risen by 200% in the last 12 months to 152, according to figures seen by Channel 4 News.

In my opinion dosette boxes, when used inappropriately, limit patient’s ability to take charge of their own health and medicines because they often just rely on what the pharmacy put in them but don’t know what they actually take. I see this regularly when doing meds recs.

I see it first hand as an ED pharmacists when patients are re-admitted to hospital with an error on a previous discharge. It’s such a vital process
"Hospital discharge prescriptions are complex & present significant risk of medication errors." This study highlights why pharmacist medication review is vital BEFORE discharge. Very hard to verify/correct prescribing discrepancies/errors after discharge!

100% agree, which is why I love being an ED pharmacist. Most interventions are prospective and the high acuity makes it more challenging but rewarding at same time. There are many roles that still need to happen on back end wards eg meds op, ED/AMU are necessarily best place

Good summary although NICE has slightly tweaked the guidelines to state to now use ORBIT tool instead of HASBLED and to initiate DOACs first line (ie don’t give patients a choice) and only give VKAs when DOACs are contraindicated

I had something similar oncall once but it was dominoes. I said “did you want the oncall pharmacist??” and they said “no this is dominoes” 😅
Umm so I got the page below. Called back and heard the lady yell “ANYONE PAGE PETER FROM GI” and some people yelled back - but didn’t sound like a nursing station.
I asked her where she was at.
