The journey of the ureter over the psoas muscle is very well seen in this sagittal CT image. This is the reason why sometimes we can’t advance proximally. Lower lithotomy and stiff GWs can help align the ureter.

Endourology is a delicate field. pt was eventually planned for
#miniPCNL
for 25x15 mm stone. BUT during placement of GW/cath for fluoro, we noticed a small papillary tumor inside the ureter. Bx taken, tm base lasered, stone surgery postponed. Always do a diagnostic endoscopy!!

The surgical practice started last year in the hospital but we had some bureaucratic issues to initiate PCNL.

Don’t force an encrusted stent with a cystoscope. Try another option & be ready for perc punture. Stay on the safe side. Always warn patients about the stent-related long term complications.

we did 2 BipoLEPs using
@OlympusUrology
enucleation probe which i found very useful. I resected the adenoma while attacted to the bladder neck b’c I don’t have a morcellator. I am very happy w the result though. Enucleate whenever you can!
