I still do. The risk of biloma is small, but if you get one, a percutaneous drain placement after a right lobe donor has a difficult window and needs readmission. Simple thing to do, and I pull them out on pod4 if drain bilirubin is not significantly elevated.

In my mind, early artery stenosis (<2w) means twisting, kinking, redundant artery and/or external compression - all of them fixable w reoperation and ensuring long term patency rather than risky stents that won’t fix the actual cause of the problem.

Nice case and video! When thrombectomy is not an option I find SMV jump graft w donor iliac vein works the best. For some reason in Wisconsin and Chile I’ve needed to do it often. Very rare cases SMV is not suitable, we think of varix/collaterals/LRV
