Thank you, @hammy_oh! Great question. The variance in C-terminal COL6A3 and endotrophin levels is not explained exclusively by BMI (no circulating protein is). Importantly, BMI and BFP (another proxy for obesity used in the study) are continuous traits, and obesity is not binary (BMI of 30, 40, and 50 represent different levels of obesity). So yes, endotrophin levels vary even in individuals without obesity due to BMI and other factors. Step 2 MR (C-terminal COL6A3/endotrophin → CAD) does not depend exclusively on obesity. The analysis suggests that endotrophin is causal for CAD in the general population, highlighting broader implications. A useful analogy can be blood glucose levels. Obesity may increase glucose levels and the risk of type 2 diabetes, but the glucose-to-diabetes association is not exclusively dependent on obesity.
