What’s right? 10K relocation stipend for each PGY-1. Guaranteed minimum salary of $100K for PGY-1 $110 K for PGY-2 $117K for PGY-3 No fellow should be making less than $125K. Residents and fellows add too much value to the bottom line of the hospital system.

@DrDrAbhiKole Yep. Read about a bill being introduced in California, which would slash 25% of investors’ profit. Don’t really know how much this would deter them. We visited some new subdivisions in and around Austin and builders said that investing firms were not allowed in. This might help.

Please explain the rational for a residency trained physician from another country to start as a PGY1? A specialist must not start as a PGY-1! This is an insult to medical education! Fellowship or a different process should be in place to complement existing knowledge!

All unmatched docs need to come together through the many unmatched advocacy organizations to hold a summit where Legislators, Hospital Admins, GME leaders do convene in one room (physical or virtual) to layout a plan forward. Inaction is unacceptable! The staffing issue is real

With 1000s of MDs/DOs graduated without a job, there are plenty of positions that unmatched docs or docs transitioning to PG training can do. These jobs only require knowledge of the healthcare system. These are good paying jobs.

The GME Extortion explained: Each year, thousands of docs are denied the opportunity to train in residency. When a doc goes unmatched (not advanced to residency), he/she must wait for another year to re-apply for the next cycle of “Match”. The loan grace period quickly shows up.1

Why is there a YOG on residency filter? Does a doctor stop being a doctor as the yr passes? Many unmatched docs take step 3 just to improve their odds of matching. Why does a doctor with yrs of experience in his/her home country is compelled to repeat residency and start as PGY1?

You look at the academic trajectory of the docs who are SOAPn, then you ask yourself why? This is not a about entitlement! You get applicants who applied to 200+ programs, cleared step 3, applied only to a primary care specialty, 3+ yrs of research, then what else to prove?

Soon, thousands of residency applicants who have passed step 1, 2, and many with step 3 will be BLOCKED by this GME system. Many of these applicants would have applied to at least 100 programs. Assume 7K (generous), that is 14,000,000 collected to apply for a job! Is that OK?

Here is what I do on Sundays afternoon. I teach real estate photography to young underserved folks at no charge! After just a few sessions, they start charging realtor $135-$245 per home. Then $360/property! That is 1hr to 1.5hr of work! Uplift others! Blessed!

@mahad_minhas Talked to a colleague about that earlier. An overwhelming percentage applied to one of the primary care specialties. Based on health data, Hispanics, Black/Brown and poor White folks have worst health outcomes. The system is just cashing in on nice talks about the underserved.

Before residency, one must pass the United States Medical Licensing Examination (1 + 2). Step 1 is 8hr long and cost US med students $645 but International Medical Grads (IMGs) $985 for the same exam and at the same center. Step 2 is 9hr and costs US grads $645 but IMGs $985. End

Arizona is doing its part and issuing a 1yr renewal training license, which pairs unmatched docs with fully license docs. But it is tough to find collaborative doctors! The same is true for Missouri! In MO, APs are going through hell just to find collaborators!

Let us say that the GME system increase PGY-1 salary to 100K a year…..an average increase of 50K for 40K residents would be about $2B. This is a great investment. You boost moral with this move. Let us say you add $10K for automatic housing stipend to each resident, that is 400M

@hyunjeankim Let us break the numbers. 80hrX52wks= 4160HRS. Allocate 160hrs for vacation time and with, let say, a salary of 52K/yr, we are now talking about residents making 13bucks/hr. Shouldn’t residents be compensated at overtime rate for each hr above 40? This feels like a conspiracy!

Arizona and Missouri have issued training/limited license which pair unmatched docs or docs in transition to GME. These legislations keep these grads in healthcare and not in food delivery like uber eats. The main roadblock is finding collaborating physicians/clinics/hospitals.

Loving it! 8-5, don’t take my job home, and weekend is for my family. I enjoy knowing that I am part of discussions, which typically lead to thousand of lives being changed!

Wouldn’t it be nice if AAMC/ERAS/NRMP to offer massive discount to re-applicants? The medical community continues to allow these corporations (hidden under non-profit status) to profit on struggling medical grads. These medical organizations are proof that medicine is a biz.

A limited license to stay in the clinical setting is the right thing to do! Are you enjoying that your fellow docs are left with no other choice but to work for Uber, Lyft, and construction company? Medicine is too toxic! It doesn’t have to be this way! Be a mentor!

Why does residency entrance occurs once a year? Is it be possible to continue channel unmatched applicants to the system throughout the year? Do people only get sick once a year? Does Sallie Mae stop calling? What about the interest rate? Do those stop?
