So we have a difficult time ahead of us. Please keep your circle of contacts as small as possible. Wash your hands. Keep your physical distance (back to zoom quiz’s on a Saturday night). And if you can’t keep your distance wear a mask. Each effort counts.
monkeypox - a short thread.
Firstly a huge thank you to the people coming forward with symptoms, clinicians that are taking samples and helping us find cases, health advisors and health protection teams for tracing contacts, vaccination teams and inpatient management teams 👏
A short thread on Group A Strep (GAS), invasive GAS (iGAS), scarlet fever and other conditions.

Tomorrow step 3 of the roadmap commences. This is the biggest step yet and even more uncertainty on how much cases will rise with the B.1.617.2 circulating in our communities. But there are things we can all do to keep case numbers low. 1/3

Thanks goes to them both for their tireless work 🙏🏼
I ❤️ this. We, anyone who works in healthcare, will always be happy to treat people when you come forward. For COVID vaccinations or anything else. That is our job. That is why we do what we do. We do not judge. People are hesitant. Come when U R ready. The sooner the better!
Can I just say, if you're unvaccinated, and change your mind but worried about people judging you at the vaccination center, this will absolutely not be the case.
We will welcome you with open arms.

Merry Christmas one and all. Thank you all for your support over the last 12 months. Hope everyone working in health, social care and public health manages to get a break as the work we do will continue 24/7 on

Hurray. My Dad received his COVID vaccine today. Well done Ireland. Now waiting for my mum to be called. And then I will feel calmer about their safety than I have done for the last year.
1. Work from home if you can
2. Keep your contacts as low as possible
3. Do regular asymptomatic LFD tests
4. If meeting people indoors in peoples houses, open windows and doors and let fresh air in
5. Wear a face covering as much as possible
6. Wash your hands regularly
2/3
7. Take a PCR test if you have COVID symptoms
8. Isolate with your household if you test positive
9. Give names of all contacts and places visited when asked by contact tracing teams
10 FINAL AND MOST IMPORTANT. Get vaccinated as soon as you meet the age, or other criteria
3/3
Thank you Tulio and all South African scientists, researchers and government for rapid sharing of your data and insights. It helps us all. Supporting countries to increase sequencing, have labs to grow virus and study, epidemiologists to deliver studies essential. 👏👏👏
Busy day on B.1.1.529 - a variant of great concern - The world should provide support to South Africa and Africa and not discriminate or isolate it! By protecting and supporting it, we will protect the world! A plea for billionaires and financial institutions. 1/8 tweets
Agree. We are all hoping but we just don’t know. Underlying immunopathology remains uncertain. What is clear is being vaccinated & boosted is essential for our own health. preventing transmission essential for all.
1. Vaccinate
2. Mask
3. Reduce contacts
4. Ventilate
5. Hygiene
For those asking whether this is good/bad news, simple answer: I don't know. While omicron may infect the lung cells less efficiently, a higher viral load may worsen immune response. We discussed the pathogenesis w/

In this pre-print, which is under peer review, we demonstrate that 2 doses of vaccination on top of prior of infection, significantly reduces both asymptomatic & symptomatic infection compared to infection alone - Table 3 - the longer since primary infection the more effect 2/6
Festival revellers returning home tonight. I hope you all had a great time. Now take a few steps to protect others on your return.
1. Access asymptomatic testing and do a test regularly (at least twice this week)
Thank you to everyone who has messaged me over the last couple of days. grateful for all of your support over very many years & especially the last couple. This honour is thanks to all of you in public health & infection community who are tireless in your service and dedication.

Infection rates are up, hospitalisation rates have increased, more suspected outbreaks are reported. Actions you can take. 1. Get vaccinated if you are offered 2. If you have respiratory symptoms, stay away from those at higher risk of severe disease AND mask up

Who else wants an internationally agreed COVID variant nomenclature that is stable over time and describes significant changes better? Similar to storm nomenclature designated by first country. But needs to be consistent, non country specific, consistent and understandable.

When to wear a fluid repellent face mask (FRFM) versus FFP3 mask - poster below. Health and safety exec advise that FRFM can be worn TIL damp and FFP3 for at least an hour (ie more than one patient) But once removed from face (eg to have a drink) then must be disposed safely.
If they hadn’t come forward and if clinicians had not recognised something unusual we would still be in the dark with no possibility of preventing spread. This is sexual health and public health in action.
Can’t believe how far we have come in 13 months… LFDs are not perfect but a great tool our population can use to
1. Test frequently at home
2. Safely allow people to not isolate as a contact of a case
3. Allow people out of isolation early safely

This is false. Vaccines not only prevent infection. They are even more effective (above 90%) at preventing severe disease, hospitalisation and death. Get vaccinated.

Can you deliver to everyone who has had to claim benefits? Key workers are still getting paid! Those who have had to claim universal credits might value a free meal.
iGAS is a sepsis syndrome so if your child is dehydrated, is very drowsy or floppy or you are worried (parents know when their child is not themselves) seek medical help. It can be difficult to assess how sick young children are and parent's judgements are important. 4/8
The Government has decided today that from midnight tonight, the entire county and city of Dublin will be placed on Level 3 of the Framework for Restrictive Measures. Dublin will remain at Level 3 for a period of 3 weeks, until Friday 9 October.

As more people vaccinated we will see more deaths in vaccinated individuals. The key is that the vaccine is preventing cases, hospitalisations & deaths. We know that vaccination is preventing hospitalisations by over 90% (95% CI 78-98%). Drugs eg dex, toci & soon mAb will help.
From June to August, many people were saying that the virus went away by itself, its got milder, nothing to worry about. All of a sudden the virus has changed again. My view.

Tips to prevent Covid-19 transmission in the workplace. When you have finished at a workspace clean the high touch areas before someone else sits there. Be that at a desk, bus drivers changeovers (steering wheel, door handles etc). Also carry hand wipes or alcohol gel and use it!
Severe disease & death is an extremely rare outcome from GAS , we are highlighting the symptoms to parents & healthcare practitioners to ensure they know the signs.
1. Scarlet fever is a rash syndrome associated with scarlet fever - feels like sandpaper & needs antibiotics 3/8
From typing of samples sent from NHS labs to
@UKHSA
, we are seeing the same strains that were already in circulation. No evidence of change though deeper analysis ongoing. Rises may be due to reduced immunity from last 3 yrs and high levels of viral co-infections circulating. 7/8
Penicillin = treatment of choice - GAS remains fully sensitive and using Penicillin mean that children are getting the right treatment while having minimal impact on other bacteria. Pls take a throat swab if you can and if suspect iGAS, needs ED assessment & blood cultures 5/8

An important study to understand the effectiveness of antivirals at scale in the UK. If you know people over 50/ over 18 with eligible health conditions then highlight the study. The sooner after test positive the better. The faster people randomised the better.
#panoramic
Have you tested positive for COVID-19 with symptoms starting in the last 5 days? Are you aged 50 or over? Or aged 18+ with an underlying health condition? Then you could be eligible to join the PANORAMIC study looking at new COVID-19 antiviral treatments
Good infection control is essential - at home, in schools, in healthcare
Wash hands
Cover faces with coughs and sneezes
Stay at home if you can [for GAS recommend that people stay at home until 24 hours after starting antibiotics]

Another round-up of London hospital data. TL;DR: still no clear sign of a changed severity profile for omicron, but it remains early days – while omicron is already dominant in London 18-64 cases, it’s probably only a third of recent admissions and even lower for inpatients.
