Yep…I’ve said this and taught this for years. But fact is asymmetric internal air cell size is normal…and the only way you can conclude that internal septae are missing is to have prior CT showing septations present. I submit that if I showed a normal good radiologist many cases of opacification, they could not tell which ones were “coalescent mastoiditis” clinically vs serous effusion vs csf leak. I agree that other complications matter. My point is that otomastoiditis is an infection and needs to be treated until it resolves….image to look for complications…call it coalescent or iridescent…but let’s not pretend we see something that’s missing when it’s so variable

Can you show the axial T2 images from L5-S1 into the presacral space.? I wonder about if extraforaminal ipsilateral L5 nerve (or nerves) are enlarged and chronically edematous. If so, it may be the L5 nerve sign, which to me implies chronic compressive neuropathy and may indicate irreversible nerve injury. Surgery may not help in this regard.

It is important to remember that most radiology conferences will be using wide format projection for power point (or other) slides. It is important for radiologists to use that format and use the space available on the slide and screen, especially to demonstrate detailed features of an image. Can you imagine a Hollywood movie director deciding not to use the entire available screen space of a movie theater? And always consider what the slide will look like in a giant room, from the back of the room, from the side, etc., not just what you see as you sit in front of your own computer while creating the presentation.

Sad to hear the news of Sherif Osman, a former fellow and friend. He was a good soul and will be remembered for always giving and caring for others. Rest in peace Sherif.

It is with a very heavy heart 🥲 that I mourn the loss of my beloved friend Dr. Sherif Osman, whom I considered a mentee and younger brother who passed away suddenly. He was an easy going and humble colleague; he was eager to keep taking care of his parents working one week in East cost to be close to JEK airport so he can travel in his two weeks off to take care of his old parents (that’s how good he was). As an immigrant who does not have close family members in US, I consider such friends, as a big family for me, so Sherif’s loss for me is like losing a first degree family member. This sad news should serve as a reminder for us to stick to each other and stay united as one big family.
