Looks like Boerhaave Syndrome - ruptured esophagus secondary to high pressure (due to severe retching due to alcohol/etc).
I remember it by thinking, the emesis/high pressure BORED through the esophagus, aka BOERhaave 🤓
I’m excited to share my *first* first-author paper was published today!
Grateful to my mentors for providing the autonomy to put this together and hopeful that it will have a positive impact on pts experiencing similar rare AEs.

Benign vascular tumor of blood vessels of skin and liver. Present at birth and often regress in childhood.
Contrasted with angiosarcoma, which is an aggressive malignant proliferation of endothelial cells of skin, breast, and liver. Associated w/ PVC/arsenic/Thorotrast exposure.

“Apple Core” lesion on barium enema x-ray suggests colorectal cancer.
High suspicion in iron deficient males (esp > 50yo) & post-menopausal women. Can use CEA as a tumor marker for monitoring (but not for screening of colorectal cancer).

Pancreatic pseudocyst! A late complication of chronic pancreatitis presenting 2-3 weeks after with persistently elevated amylase. Characterized as a pseudocyst because it does not have a true epithelial lining. Complications include rupture!

I’d say mixed connective tissue disorder. The sicca makes me think sjorgens (likely secondary), Scl70 suggests diffuse systemic scleroderma, and the stocking glove could be due to polymyositis. Would be interested in seeing the anti-U1RNP result!

I am seeing the comments it's a downward nystagmus, but I thought we determine the nystagmus based on the fast phase, so wouldn't this be upward nystagmus?
Also, vertical nystagmus would suggest brain pathology versus horizontal nystagmus suggests inner ear/ CN VIll pathology.

Don’t give up! Have you looked into postbac programs that offer guaranteed admissions to medical school? Many programs have guaranteed interviews with the med schools, but there are a few with guaranteed admission if you perform well and successfully retake the MCAT.

This is helpful! I learned that napkin ring lesions were only suggestive of left sided colorectal carcinomas, but looks like “apple core” and “napkin ring” are interchangeable. Thanks for sharing!
