Respiratory Epithelial Adenomatoid Hamartoma (REAH) Synonyms: Glandular hamartoma; nasal hamartoma Benign acquired nonneoplastic overgrowth of indigenous glands of the nasal cavity, paranasal sinuses, and nasopharynx arising from the surface ciliated respiratory epithelial cells surrounded by a thickened basement membrane and devoid of ectodermal, neuroectodermal, and/or mesodermal elements. Clinical • Predominantly occur in adult patients, with a decidedly male predominance; patients range in age from the 3rd to 9th decades, with a reported median age in the 6th decade of life. • Most occur in the 👃🏽 cavity, in particular the posterior 👃🏽 septum: ○ Involvement of other intranasal sites occurs less often; Lateral 👃🏽 wall, middle meatus, and inferior turbinate. ○ Other sites; the nasopharynx, ethmoid sinus, and frontal sinus. Etiology: ○ No association with any specific etiologic agent ○ Arise in the setting of inflammatory polyps, raising a possible developmental induction secondary to the inflammatory process • Pathogenesis: ○ Increased fractional allelic loss (FAL) of 31%, falling in between an FAL of 1% for chronic sinusitis and 64% for sinonasal adenoca: ‐ Based on the appreciable allelic loss within hamartomas being considered unusually high for a nonneoplastic entity, the possibility that REAHs may be a benign neoplasm rather than a hamartoma has been suggested.
