PAs are replacing doctors in some regions/settings rather than complimenting their role. Pay disputes ongoing almost 2 years after negotiations broke down. Foundation programmes in a constant state of flux. And people wonder why doctors are leaving? It’s an embarrassment.

From a surgical perspective, I fear that unless we address the sheer cost of CST to the trainee to achieve all the necessary "educational" tick box components combined with the heavy service provision and lack of true training opportunities, numbers will continue to dwindle.
Only 37.7% of F2s went straight into specialty training in 2018. That's down from 42.6% last year, and 71.3% in 2011. More and more F2s still don't see CT/ST as a desirable option straight away.

I agree with the jumping through hoops comment. This is relevant to all trainees however UKG are told this from undergraduate level therefore have more notice. The other issue reported is the divide between international training and U.K. exam structure.

Excellent paper Rhi. As mentioned, at least £20k to CCT and that is a conservative measure based on doing the minimum requirements for CCT not necessarily what is required to be competitive for a consultant post. Also didn't realise that the UHB get £12,400/trainee/year 🤔
