Really concerning as I know so many interventional cardiology fellows in UK doing additional research fellowships because of lack of job opportunities and NPs are doing TAVRs just because we want to cater to their egos. There is absolutely no reason this should be happening.

this is so inappropriate on so many levels. Being a TAVR operator is not just about deploying a valve, but working up the pt for the optimal mx of their AS, managing the patient prior to procedure and dealing with complications. This is not safe for patients

Guide is deep seated. Avoid contrast. Wire the lesion. IVUS to confirm true lumen position and identify distal landing zone/sizing of vessel. Stent. IVUS again before any other contrast injection to ensure no distal/spiral dissection
Breaking bad news to attending: While informing attending about the complication of a case, always say something positive to take the edge off. Like patient was denying chest pain or was enjoying movie, while we were holding pressure on large groin hematoma.

In our hospital system, we have back up system in place if residents reach cap. As a resident who just finished training, I believe it’s also a safety issue. If a resident admits more than 10 patients, he/she misses important stuff. Also not good for learning
It’s unfortunate that Pakistan couldn’t win the final, but nevertheless a well contested tournament came to an end. For me the highlight was when our program director supported Pakistan cricket team against Afghanistan

2) If you don't do intravascular imaging, don't even think about posting
3) If you do OCT, prepare for the IVUS crowd to scoff
