Case Update: As some of you have guessed, we did in situ laser fenestration to R Renal + covered stent, coiled/covered L renal because it was involved in the aneurysm and sealed bellow SMA/Celiac. Post op CT looked good. Patient went home 5 days post op with close follow up and

1-Open if good surgical candidate, clamp bellow renal and distal anast to iliac bifurcation or IIAs and jump grafts (Or just ligate L IIA).
2-EVAR with IBG on R side, coil and cover L IIA and land on EIA. Or staged embolization of both IIAs (75yo) and land on EIAs both sides.
75 years old,♀,single kidney, aortic aneurysm measuring 9.3 cm, right common iliac artery aneurysm measuring 6.8 cm and left common iliac artery aneurysm measuring 6.9 cm. WWYD?

Great question. It was indeed an extensive debate. The final decision was based on the severity of trauma, concerning aortic findings on CT (circumferential dissection), pt young age, and he already had a laparotomy for other reasons. Therefore, the group decision was to explore

Unfortunate but have seen an increasing number of aortic floating thrombus since COVID in young pts. We start them on heparin drip + repeat CTA in 2-3 days, if no improvement or worsening symptoms then surgical thrombectomy. Also, thrombophilia workup + echo + malignancy

Agree with everyone. Venous system has low flow and therefore aneurysms don't tend to rupture. I would do serial imaging & watch for DVT + prophylaxis if possible. Interested to know what could be the etiology🤔

You're absolutely right, looks like an endograft in the L subclavian with the main proximal graft flush with the carotid. In a 50 yo, presumably otherwise healthy, open debranching with reconstruction is the durable option.

Challenging case and thank you for sharing. When was the T/EVAR done? Was there a genetic testing? I think those factors would influence decision making. With a normal EF, if otherwise pt is surgically fit, and rapid aneurysmal growth (1cm/4month), I would be more inclined

That was initially my concern and it's nice to hear about other experiences. I have to say that i changed my mind after I saw results from two large volume tertiary centers using stapled bovine grafts for aortic and IVC reconstructions in infected fields.
