I had a rough time in clinic yesterday. Saw a patient with very bad likely neuroendocrine prostate cancer that quickly metsd to lungs and liver. Had a hard time balancing being honest about prognosis vs. providing hope and comfort. Do others struggle with this like I do?
This is my dad who is in a nursing home with dementia. I took this yesterday- which was the first day I had been allowed/able to see him in nearly a year. It was also the same day he got vaccinated so hopefully I can now start seeing him more regularly again.

My father is in a SNF and has dementia. In January, he knew who I was and could still speak (barely). 3 months of isolation and he has lost his ability to communicate and doesn't recognize me on facetime. I know they meant well but COVID got him anyways.

Matching observational cohort analyses can be very problematic. I can show you similar analyses in prostate cancer that favor surgery and your response will be "we need RCT data"

Yes. I totally understand why the USPSTF would recommend against this test. I mean it's VERY costly and there are REAL harms to testicular self-exam. I mean a guy might squeeze too hard and end up in the ER, right?

Didn't cry in med school. The gods of medicine saved it for my internship and made it bad. A 7 year old boy undergoing a minor biopsy procedure coded on the OR table and died because of a malfunction with the anesthesia machine. Still one of the worst days of my life...
If you missed this, read it NOW. Stop rushing to publish bad studies with the word COVID in the title...and shame on the journals for facilitating it. Everyone bears some responsibility here.
