Lesson from Hemodynamics:
Too stiff LV (HFpEF) ➡️ pulmonary edema & poor outcomes
Too accommodating (dilated) LV (HFrEF) ➡️ pulmonary edema & poor outcomes
We should be flexible enough to stay euvolemic but not too soft that any small trigger can put us in volume overload!

60 Y AA obese M coming in with AMS, shock and AKI getting treated as septic shock. My POCUS exam showed severely dilated RV compressing on LV, ScvO2-30. Started empirically on heparin drip and next day Doppler showed extensive femoral DVT.
