A simple but important observation after 20+ years performing operative fetoscopy on monochorionic twins. ALL monochorionic placentas have vascular communications between the twins. It may be difficult to detect, but it’s there. #TTTS #FetalSurgery

This first-of-its-kind technology is located in the hospital's brand new Cardiac Imaging Suite. The low-field MRI provides patients the following key benefits: 1. Provides clear heart imaging in a quieter, more spacious environment for added comfort. 2. Accommodates movement, making it ideal for imaging fetuses, as well as the heart, lungs, and GI tract of wiggly children. 3. Reduces the need for general anesthesia; alternative methods of pain management with fewer side effects can be used instead, such as virtual reality goggles. 4. Supports patients with metal implants, including pacemakers, overcoming limitations of traditional MRI technology. There was a time when medicine was a noble pursuit, a realm for the skilled, the driven, the brilliant. A time when physicians stood as masters of their craft, when innovation was bound only by the limits of human ingenuity. That time is gone. In its place stands a grotesque monument to mediocrity, built not by healers but by bureaucrats, middlemen, and political charlatans. A system where the incompetent dictate to the competent, where those who produce are shackled by those who leech. They call it healthcare. It is not. It is a cartel. A machine engineered to extract, to obscure, to consolidate power in the hands of those who would not last a day in the operating room, the emergency room, the laboratory, or the trenches where real medicine happens. The grift is staggering. Trillions wasted. Prices inflated beyond reason. Access rationed, not by merit, not by innovation, but by bureaucratic decree. The patient is not a priority, only a number, a pawn in a game of financial manipulation. But there is a weapon against this decay: transparency. The parasites fear it. They know that once the numbers are visible, the illusion crumbles. They know that when patients see the truth, when physicians are freed from the yoke of opacity, their reign ends. The choice is clear: Expose them, or be ruled by them. Tear down their fortress of fraud, or resign yourself to servitude. There is no middle ground. Let the light in. Let the truth be known. And let those who heal, who create, who build finally take back what is rightfully theirs. #healthcare

We did see the patient without authorization. That’s when we diagnosed the dual fetal demise. Insurance authorization is required for office consultation (which I waved) as well as the hospital stay and surgery. Finally, a fetal emergency is not recognized by insurance companies. The mother appears fine. The babies are sick based on ultrasound evidence. #TTTS

The saddest and most tragic update. Both twins are dead. Parents are devastated. We were unable to do the life saving fetal surgery due to lack of insurance authorization from LA Care. #LACare

We agree. Monochorionic twins with isolated polyhydramnios or isolated oligohydramnios are at high risk. Here are a couple of studies we published on this topic a few years back.
