@psypharmacopeia History of Psychiatry I loved it. It's why I know about the "anti-psychiatry movement" from circa 1960s. Is that what you're referring to?

@drjohnm @doc_BLocke 10/100,000; "rare"? Depends on the competing alternatives for esophageal perforation/injury. @contextualinformation

@doc_BLocke If I could program the initial set of commands into every #internist it would be: Get a complete or at least an ample history, especially background context Formulate a #differential #diagnosis Those two commands alone, faithfully executed, would solve a lot of problems

I cut corners like everyone else. But usually before I consult someone, I do a lot of the foundational work myself And when I am the consultant, I do my best to give the patient and the team their due. Sometimes that means bronchoscopy Sometimes it means explaining why bronchoscopy is a low value

Recently had a general surgeon refused to do an excisional biopsy of a lymph node for a high probability diagnosis of recurrent lymphoma There was some pathetic excuse about maybe it was recurrent breast cancer instead, which of course wouldn’t have made a difference. The real reason: OR schedule and Thursday meaning an add on Friday case It’s OK, we found lymphoma a week later on bone marrow biopsy

@theblanketdog How is that different than somebody wanting to use the robot in order to justify its cost? It is still systemic irrationality, which was the point of my tweet

@theblanketdog I’m not being sarcastic. The willingness of consultants to oblige reasonable requests is not uniformly high across providers & institutions & day of week & time of day & holidays You only have to hear unreasonable excuses so many times before you begin to expect them

An alternative is that somehow the infection precipitated the rash through immunological means but that seems like a stretch also So, this perhaps is best classified as a rare instance of two diseases happening to present symptomatically at the same time; category four in our classification scheme

@AdamRodmanMD Is this because, contrary to prior tweet, AI dose have in the training sets tribes of data on human emotional reactions even in fictional literature?

@AdamRodmanMD Is this because AI training sets don’t contain info on how or why most tests are ordered, and drugs prescribed? That’s my guess
