I cut corners like everyone else. But usually before I consult someone, I do a lot of the foundational work myself And when I am the consultant, I do my best to give the patient and the team their due. Sometimes that means bronchoscopy Sometimes it means explaining why bronchoscopy is a low value

Recently had a general surgeon refused to do an excisional biopsy of a lymph node for a high probability diagnosis of recurrent lymphoma There was some pathetic excuse about maybe it was recurrent breast cancer instead, which of course wouldn’t have made a difference. The real reason: OR schedule and Thursday meaning an add on Friday case It’s OK, we found lymphoma a week later on bone marrow biopsy

@theblanketdog I’m not being sarcastic. The willingness of consultants to oblige reasonable requests is not uniformly high across providers & institutions & day of week & time of day & holidays You only have to hear unreasonable excuses so many times before you begin to expect them

An alternative is that somehow the infection precipitated the rash through immunological means but that seems like a stretch also So, this perhaps is best classified as a rare instance of two diseases happening to present symptomatically at the same time; category four in our classification scheme
