Been doing many telephone clinics recently. I always start by apologising for not seeing people face to face, to which the response is usually “don’t worry doctor I totally understand, thanks for ringing”. Nottingham patients are, on the whole, awesome.

Assuming I make it through tonight, I will have survived exactly 10 years as a Consultant. It has certainly been an eventful, and for the most part, enjoyable time. Plenty more stones to be lasered though - Bring on the 2020s!
If I had a brain cell for every time someone trying to contact a neurologist accidentally gets put through to me, I might actually have enough to be a neurologist.
Ureteric stents: Giving patients information about stents is SO important and can help them understand the potential side effects of stents, how soon stents to be changed/removed and how to seek assistance if there are problems.
Some proper stones today, no SpR in theatre so I got to do it all myself too - bonus 😀 (much as I love training, it is good to keep your hand in occasionally...)

Reasons apart from Covid:
1. Fewer normal coughs and colds

This has resided in the office in main theatres at Nottingham City Hospital since I was a student. I remember staring at it whilst waiting to do my first solo appendicectomy as an SHO, and now 25 years later waiting to do a primary URS. I think it’s still relevant and wise.

Finding assessment of male LUTS, which are highly subjective symptoms very difficult via remote consultation. Ideally need to do a DRE, check flow rate, PVR etc. I'm thinking it's almost not worth having a remote consultation with these chaps.
It’s been a long, hard few months for the candidates but the FRCS(Urol) vivas are finally here. Good luck to all of you. Stay calm, trust in your knowledge and preparation - go in and smash it!

It feels like it’s taken forever but we now have LASER BLINDS IN EMERGENCY THEATRES!!!! (Have had the actual laser for ages). Hot ureteroscopy access in Nottingham is one stop closer. Hopefully will be able to start next week.

After 2 middle grade night shifts week, I think that short (so called “female”) catheters ought to be banned. No problem putting a longer catheter into a woman but horrendous when a short catheter is put into a man and the balloon inflated in the prostate…

Did our third *totally* tubeless MIP-S (12Fr sheath) PCNL for 16mm calyceal stone yesterday. Stone free and home day 1.
I reckon that I wouldn't have had the same result with RIRS - more than likely residual frags and stent!

6. Very detailed metabolic evaluation is an option for some recurrent calcium stone formers, BUT for the vast majority, the evidence supports empirical treatment, namely
-More water 💧
-less salt 🧂
-less animal protein 🐮
-more fruit and veg🥕🍎
