@juliaisobela 8 hrs into an all day surgery, I projectile launched about 100 cc of purified fat up to the ceiling (not over a sterile area) and it splatted SO loud. Surgeon laughed so hard he broke scrub. The fat stuck…and hung there for the remaining 2 hours. CRNA air dropped me this pic 🤦🏼‍♂️

@empanicturtle @DrLongissimus I think we’d make it pretty far. We’re skilled in dealing with aggressive belligerent zombie like people already. We carry enough sedatives, paralytics, and miscellaneous drugs to buy time, and a solid 2/3 of items lying around can be macgyvered into projectiles. We good.

Observations after 1 month of being an attending… • this is so much better than residency but still v hard • being a baby attending in a group of mostly veteran EM docs makes me feel slow • holy crap the decision fatigue • pre shift anxiety/terror is strong

@EM_RESUS Similar case early in PGY2. Pt had used meth, subsequently developed crushing retrosternal pain. ECG not as striking (repeats more impressive) but my attending and I called a STEMI alert. Much of cath team scoffed at us but interventionalist agreed w/ me & took the pt… 99% RCA

An ECG like this, regardless of substance use, MUST be considered acute ischemia unless and until proven otherwise!! Great reminder that amphetamines can do more than just cause coronary vasospasm…they can cause plaque rupture and acute MI. Thanks Dr. Ghali!

@EM_RESUS Probably 2:1 flutter in the setting of LMCA STEMI. Initially thought VTach with left main STEMI but the more I look at it the less I think that. Nonetheless…shock him, activate STEMI alert, and run like hell to the cath lab.

@chrissyfreeMD Facts. I’d also say that loving what you’re doing and the feels of exhaustion and being stretched thin are not anywhere close to mutually exclusive. Those who don’t find it brutal are the exception not the rule. I wouldn’t want any other job, but god damn I’m tired.
